Clinical Effect Of Surgical Repair Of Ankle Deltoid Ligament Injury

Objective: to observe the effect of surgical repair in ankle deltoid ligament injury.Methods: from September 2014 to April 2017,18 patients with ankle fracture associated with deltoid ligament injury were selected from Department of Orthopedics,General Hospital of Ning Xia Medical University,including 11 male patients and 7 female patients.The age ranged from 18 to 52 years old,the average age was 39.4 years old,8 cases were left ankle and 10 cases were right ankle.According to Lauge-Hansen classification,there were 8 cases of supination-lateral rotation type,7 cases of pronation-lateral rotation type and 3 cases of pronation and abduction type.Preoperative MRI examination of ankle joint in diagnosis of deltoid ligament ruptured.All the lesions of deltoid ligament were treated surgically to repair.The patients were followed up for 1 years before and after operation,and then the gravity stress X-ray examination was performed again.The medial malleolus space and the medial malleolus space were statistically analyzed.The clinical effects were evaluated according to the ankle back foot score and visual analogue score(VAS)of the American Department of orthopedics at the last follow-up(AOFAS)to understand the postoperative recovery.Results: All 18 patients achieved anatomical reduction.The average healing time was11.8 weeks(9~15 weeks)without delayed union.All the wounds healed by grade A,without incision infection,no screw loosening,no vascular nerve injury and osteomyelitis.After 1years of follow-up,X-ray examination showed that all the fractures healed and the medial malleolus space of ankle joint returned to normal.1 patients were followed up at 1 years after the operation,and the feeling of the ankle was tight,but it did not affect the movement of the normal ankle.The score of AOFAS ankle-hind foot in the last follow-up was 92.6 points(74-97points),the excellent and good rate was 94.4%.VAS score was 0-4,the average scorewas 1.3.The preoperative medial malleolus space was 5.06 ± 0.21 mm,which was significantly higher than that of the postoperatively affected medial malleolus space of 2.56±0.13 mm.Medial malleolus space 1 year after operation There was no significant difference between 2.62 ±0.12 mm and 2.59 ±0.11 mm in the healthy side.Conclusion: The fracture of the ankle combined with the damage of the trigonometric ligament,the fracture is fixed in the reduction and internal fixation.The repair of the fractured trigonum ligament in the same period can make the rehabilitation of the ankle joint function better,and the stability of the ankle joint can be recovered well.
