| Objective: To summarize the clinical manifestations of pulmonary aspergillus,to investigate the diagnostic value of the bronchoalveolar lavage fluid culture and improve the understanding of pulmonary aspergillosis.Methods: 54 cases with bronchoalveolar lavage fluid culture positive of Ningxia Hospital from January 2012 to March 2017 were collected.According to the inclusion and exclusion criteria,the indexes of 45 patients with pulmonary aspergillus including the clinical features,laboratory test,imaging findings and bronchoscopic performance were analyzed,Results:1.36 cases had basic diseases,27 cases with chronic pulmonary disease,12 cases with diabetes,6 cases with tumor and 3 cases with autoimmune diseases.2.Clinical symptoms included cough,phlegm,fever,shortness of breath,wheezing,chest tightness and other respiratory tract infection symptoms.The imaging findings included solid lung changes,bronchodilatation,pleural thickening,pleural effusion,single or multiple holes,double track sign,crescent sign,halo sign,and so on.3.Sputum culture was observed in 38 cases.Among them 20 cases of sputum culture positive,13 cases were aspergillus,5 cases were bacterial,and 2 cases were aspergillus combined with bacterial.4.43 cases were abnormal under the bronchoscopy.40 cases were aspergillus,and 5cases were aspergillus combined with bacterial.5.Of all the 45 patients were treated with antifungal therapy,39 cases were relieved and improved,4 cases were worse,and 2 cases died.Conclusion: Among the 45 cases of aspergillus infections,most of them had basicdiseases.The clinical manifestations and imaging results were unpecificity.Aspergillus has a variety of manifestations under the bronchoscopy,and It is necessary to obtain the etiological basis of BALF test. |
