Objective To analyze the relationship between the number of lymph node examined and rN stage on thoracic esophageal squamous cell carcinoma.Methods To collect 168 patients with thoracic esophageal squamous cell carcinoma who underwent esophageal cancer radical resection and to register their clinicopathological data and establish a database.Retrospective analysis of the number of lymph node examined and rN stage on the prognosis of patients who with thoracic esophageal squamous cell carcinoma.Results 1.Among 168 patients with thoracic esophageal squamous cell carcinoma,106 patients were lymph node positive and 63 patients were lymph node negative.Multivariate analysis showed that histological type and invasion depth were significant factors associated with lymph node metastasis(P<0.05)..2.When the number of lymph node examinations was 11.5,the largest Youden index was obtained.The 5-year survival rate of ESCC patients in the Lianying 2 group was 64.6% vs 19.4%,a statistically significant difference was observed(P<0.001).There was no statistically significant difference in perioperative recovery.(P>0.05).3.Use the ROC curve was drawn with rN staging and pN staging,the diagnostic value of rN staging is higher than pN staging.The average follow-up time of 168 patients with thoracic esophageal squamous cell carcinoma was 38.89±22.138 months,and the 5-year overall survival rate was 45.2%.Without rN staging: Cox proportional hazards model analysis showed that histological type,invasion depth,pN staging,and lymph node examined as independent prognostic factors.Inclusion of rN staging: multivariate analysis showed that: histological type,invasion depth and rN stage as independent prognostic factors.Conclusion 1.The degree of differentiation and depth of invasion were independent risk factors for lymph node metastasis in thoracic esophageal squamous cell carcinoma.2.The number of lymph node examined was significantly correlated with the survival time and the sensitivity of pN staging of patients with thoracic esophageal squamous cell carcinoma.it examined did not affect the perioperative recovery of patients with thoracic esophageal squamous cell carcinoma.In addition,the number of sentinel lymph nodes should meet at least 12 during radical surgery.3.rN staging is an independent risk factor for the prognosis of thoracic esophageal squamous cell carcinoma.It can be used as a sensitive index for postoperative staging and prognosis of patients with thoracic esophageal squamous cell carcinoma,especially when the number of lymph node examined does not meet 12.
