Objectives: To investigate the relationship between the extent of collagen fibrosis in the ultrasound spiculation sign of breast cancer and the prognostic factors.Method: Collected 60 cases of breast cancer patients confirmed by pathology from December 2016 to October 2017 in the General Hospital of Ningxia Medical University,preoperative ultrasound examination confirmed the appearance of spiculation sign around the mass,All breast cancer specimens were subjected to routine pathological examination and immunohistochemical examination of ER,PR,HER-2,and KI-67.The pathological tissue of the spiculation was dehydrated,sectioned,and routinely stained.The speculation tissue sections were routine stained to observe the degree of collagen fibrosis and to investigate the relationship between the collagen fibroplasia in the ultrasound spiculation and prognostic factors of breast cancer.Results:1.Among the 60 breast cancer patients that proformed to speculation around tumors in the ultrasound,50 cases of non-specific invasive breast cancer(invasive ductal carcinoma),3 cases of breast mucinous carcinoma,and 4 cases of invasive micropapillary cancer,2 cases of invasive lobular carcinoma,and 1 case medullary carcinoma.2.The pathological results of 60 cases showed that there were 20 cases of mild collagen fibrosis,16 cases of moderate of collagen fibrosis,and 24 cases ofsevere collagen fibrosis.3.There was no correlation between the degree of collagen fibrosis and the expression of PR and KI-67.4.The degree of collagen fibrosis was related to the histological grade.The high histological grade with severe hyperplasia,there was significant statistical differences between groups(P=0.009).5.The degree of collagen fibrosis was related to the size of the tumor,and severe hyperplasia with larger tumor,there was significant statistical differences between groups(P=0.010).6.The degree of collagen fibrosis was related to the expression of ER and HER-2.Severe hyperplasia with low expression of ER and high expression of HER-2,there was statistical differences between the groups(P1=0.038,P2=0.029).7.The degree of collagen fibrosis was associated with lymph node metastasis.The rate of lymph node metastasis was high in patients with severe hyperplasia.There was statistical difference between the groups(P=0.038).8.The degree of collagen fibrosis was positively correlated with the number of spiculations,with statistical differences between the groups(P=0.031).Conclusions:1.The spiculation of breast cancer is the sonogram of tumor-matrix interaction.It is the basic mechanism of tumor progression,including the proliferation of collagen fibers and the infiltration of adipose tissue.2.The degree of collagen fibrosis in the spiculation of breast cancer is related to the prognosis.Collagen hyperplasia is associated with high histological grade,ER-negative,HER-2 positive and lymph node metastasis,and the degree of hyperplasia is positively correlated with the number of spiculations in the ultrasound manifestation.Collagen fibrosis at the burr may predict poor prognosisin breast cancer.
