Objective To compare the differences volume of Peripancreatic Collections and the MCTSI in patients between moderately severe and severe acute pancreatitis, and Comparision the value of MCTSI and Peripancreatic Collections volume in evaluating the severity of acute pancreatitis.Method A total of 69 patients with acute pancreatitis admitted to our research,69 patients come from Ningxia Medical University Affiliated Hospital from January 2016 to June 2017.The 69 patients divided into two groups of 41 moderately severe and 28 severe acute pancreatitis. The volume of Peripancreatic Collections in each patient was measured, and the modified CT severity index was scored for each case. The volume of Peripancreatic fluid Collections and the MCTSI score were compared between the two groups. The ROC was used to compare the two methods to evaluate the severity of acute pancreatitis. Results 69 cases of AP patients,41 cases of moderately severe acute pancreatitis,28 cases of severe acute pancreatitis.(1)There was no significant difference in sex, age, etiology, serum lipase and serum amylase between the two groups(P> 0.05).(2)There was a significant difference between the two groups in the volume of Peripancreatic Collections(P<0.05).T value of Peripancreatic Collections is 926.00?(3)There was a significant difference between the two groups in the volume of the MCTSI score(P <0.05).T value of the MCTSI score is 951.50.(3)The receiver operating characteristic curve(ROC)was used to evaluate the accuracy of MCTSI score and volume of Peripancreatic Collections in judging the severity of acute pancreatitis. The areas under the ROC curve were AUC of MCTSI score is 0.921 and95 % Confidence intervals(0.858-0.985),The cutoff value is 6,the sensitivity is 73.2% and the specificity is 96.4%;The areas of Peripancreatic Collections volume 0.947,95%confidence interval(0.897-0.997),The cutoff value is 825.6cc,the sensitivity is 92.7% and the specificity is 85.7%.Conclusion Measurement of volume of Peripancreatic Collections fluid and MCTSI score are good indicators for judging severe acute pancreatitis, but the diagnostic efficacy of measuring the volume of Peripancreatic Collections fluid is superior to the MCTSI score.Combined with a variety of imaging methods can initially distinguish the severity of acute pancreatitis, which will distinguish between severe acute pancreatitis and severe acute pancreatitis to provide a more reliable theoretical basis for the early clinical diagnosis and treatment of severe acute pancreatitis.
