Objective To study the therapeutic effect of Hui Medicine Qingbai Santan Chaji on hydroquinone-induced experimental vitiligo guinea pigs and the mechanism of inducing pigmentation,which provides scientific basis for the wide application of Qingbai Santan Chaji in clinical practice.Methods Forty healthy black guinea pigs were randomly divided into four groups: normal group,model group,positive control group and Qingbai Santan chaji group.The vitiligo model was prepared by applying 5% hydroquinone to the back of the black guinea pig's back to the hair.Qingbai Santan Chaji group was treated with Qingbai Powder liniment,the positive control group was treated with compound Kaliperidin tincture,the normal and model groups were treated with saline,and all groups were treated with local external liniment twice a day for 60 days.Observe and record pigmentation and detect the following indicators:(1)The pigmentation of lesions was evaluated by naked eye(2)The histopathological changes of the skin were observed by HE staining and the thickness of the epidermis was measured.(3)Observation of melanin production and distribution by Fontana-Masson(4)The expression of tumor necrosis factor TNF-a and Langerhans cell marker CD1-a were detected by immunohistochemistry.(5)Determination of superoxide dismutase in skin by xanthine oxidase method(SOD)Activity.Results 1.Naked eye observation,except the normal group,the other groups were coated with 5% hydroquinone in the guinea pig skin.The positive control group and Qingbai Santan Chaji group had different degrees of pigmentation after drug treatment,but no obvious changes were found in normal group and model group.2.Under HE staining,melanocytes in the basal layer of epidermis in the model group were rare,vacuolar and other pathological changes occurred,and the thickness of the epidermis was significantly thinner than that of the normal group,and the difference was statistically significant.(P<0.05);Compared with the model group,Qingbai Santan Chaji group significantly increased the melanocytes,improved the pathological changes and significantly thickened the epidermis.(P<0.05);Compared with the model group,the positive control group had more melanocytes and no obvious thickening of the epidermis.The difference was not statistically significant.(P>0.05).3.Under Fontana-Masson method,melanin was occasionally found in skin epidermis and spinous layer of guinea pigs in the model group,and melanin in hair follicle was decreased obviously,which was significantly different from that in normal group.(P<0.01)In the positive control group and Qingbai Santan Chaji group,more melanin granules were found in the basal layer and spinous layer of the epidermis,and the distribution of melanin cap was continuous in the keratinocytes.There was significant difference between the two groups compared with the model group.(P<0.01).4.The expression of TNF-a in the lesions of the model group was significantly higher than that in the normal group under immunohistochemical staining,and the difference was significant.(P<0.01);The positive control group and Qingbai Santan Chaji group had significantly lower TNF-a expression levels in the lesions than that in the model group.(P<0.01).The number of CD1-a positive cells in the model group was significantly higher than that in the normal group.(P<0.05),And the structure and shape of positive cells were deformed.The number of CD1-a positive cells in the positive control group and Qingbai Santan Chaji group was significantly lower than that in the model group.(P<0.05),But there still exists the deformation of the structure and shape of the positive cells.5.The SOD value in the lesions of guinea pigs in the model group was significantly higher than that in the normal group,and the difference was statistically significant.(P<0.05).The SOD value of Qingbai Santan Chaji group and the positive control group were lower than that of the model group,and the difference was statistically significant.(P<0.05).Conclusion Application of Qingbai Santan Chaji can promote melanin synthesis and black cell proliferation,effectively reduce the skin melanin decomposition,improve the local microenvironment cytokine immunoregulation and maintain the balance of oxidation and antioxidation in the local environment of vitiligo.Qingbai Santan Chaji has good therapeutic effect on vitiligo guinea pig model.It is safe,stable,simple and easy to use,and can be widely used in the treatment of vitiligo.
