Different Productive Start Time Observation Of The Effect Of The Prevention Of Pulmonary Complications After Esophagectomy

Objective:To study the effect of different sputum drainage time on pulmonary infection in postoperative patients with esophageal carcinomaObject 120 cases of esophageal cancer patients with esophageal carcin-oma were chosen from January 2016 to July 2016.Methods:Selection of 120 esophageal cancer patients met the inclusion criteria were randomly divided into A group and B group,the number of eligible patients,the number in A group,B group and A group in the number of general anesthesia awake after?6 hours on average after operation?,began to take the conventional sputum nursing;B in the first day after the operation?average 20 hours after operation?began to take the conventional sputum nursing.The chest physical therapy after thoracotomy usually include turning back,cough,expectoration nursing routine in addition also includes inhalation and suction auxiliary measures.Pulmonary infection in two groups were recorded at different time expectoration nursing,atelectasis,for fiberoptic bronchoscopy The suction and the postoperative hospitalization time and hospitalization expenses,the heart rate of 48h after operation,closed thoracic drainage tube removal time of pulmonary infection were analyzed.Inclusion criteria:?1?T fever of more than 38 Degree C;?2?White blood cells over 10x10⁹/L;?3?X-ray operation the lateral lung density increased;?4?Cough,expectoration,pulmonary rales auscultation hypoxemia.Inclusion criteria:the heart rate increased more than 20%,occurrence of ventricular tachycardia,blood gas analysis of?PaO₂?<80%radial artery blood check,or arterial oxygen saturation?SaO₂?<90%.Atelectasis criteria:bedside chest X-ray X-ray showed that a Lateral atelectasis or atelectasis.SPSS l7.0 statistical software was used to analyze the collected data.The enumeration data were described by Spearman statistics.Results:1 Aulmonary infection situation:A group of 60 cases of 3 cases of pulmonary infection,B group 60 cases of 5 cases of pulmonary infection,significantly more than group A,the two groups were statistically significant difference?P<0.01?.2 Hypoxemia occurred in 5 cases of group A,and hypoxemia occurred in60 cases.There were 9 cases of hypoxemia in 60 cases in group B,which was significantly higher than that in group A,and the difference between the two groups was statistically significant?P<0.01?.3 Atelectasis occurred in 60 cases of group A,4 cases of atelectasis,and60 cases in group B showed atelectasis in 6 cases.The difference between the two groups was statistically significant?P<0.01?.4 Medical expenses:A group after medical expenses?4.64±0.53?wan yuan,B group postoperative medical expenses?5.28±0.51?wan yuan,the difference was significant?P<0.01?.5 Hospitalization time in A group?11.87±0.71?days and group B?14.2±2.51?days.There were significant differences?P<0.01?.6 Heart rate:A group patients with postoperative heart rate?90.6±6.43?bmp,B group patients with postoperative heart rate?105±6.17?bmp,there was significant difference between two groups?P<0.01?.7 Chest tube extraction time:A group?4.57±0.54?days,and B group?5.13±0.71?days,there was significant difference?P<0.01?.Conclusions:This study of 120 cases esophageal cancer patients after expectoration nursing early shows that early effective cough,eliminate airway secretions can effectively improve partial pressure of carbon dioxide in artery,increase the arterial partial pressure of oxygen,thereby improve the ventilation disorder and occurrence of arrhythmia,remove mucus retention,and promote lung expansion,shorten the closed thoracic drainage tube extubation time.So the pulmonary infection can be prevented,the patients can get uneventful recovery,and therefor get shorter hospitalization time,less hospitalization costs.
