Objective: To explore the clinical characteristics of severe hand-foot-and-mouth disease(HFMD)caused by EV71 in western area of Zhuhai city by comparing the clinical materials of various pathogens caused severe HFMD in order to provide theoretical basis for the clinical diagnosis and treatment.Methods: Retrospectively collecting the 66 HFMD cases between Jan.1 2012 to jun.31 2017 from the pediatric Internal Medicine department of zunyi medical college affiliated 5(zhuhai)and dividing them into EV71 severe group(36 cases)and other-pathogen severe group(30 cases),we analyzed the clinical materials of both groups.In addition,we also analyzed clinical characteristics of the mild and severe HFMD caused by EV71 by collecting the homeochronous mild HFMD cases(50 cases)cuased by EV71 in the pediatric Internal Medicine department of a hospital.The results were statistically processed by SPSS18.0 which indicates the statistic significance with P?0.05.Results: 1.The medians of fever term were separately 3 days and 2days in EV71 severe group and other-pathogen severe group,which indicated a statistically significant difference(p<0.05).There did not exist any significant difference in the highest body temperatures,ages and genders in both groups(p>0.05).2.The limb shaking incidence were respectively 83.3% and 56.7% in EV71 severe group and other-pathogen severe group,which showed a statistically significant differences(p?0.05);the convulsion incidence were respectively 2.8% and 26.7% in EV71 severe group and other-pathogen severe group,which showed a statistically significant differences(p?0.05);the hyperarousal and vomit did not have any significant difference in both groups(p>0.05).3.The cold acra incidences were separately 44.4% and 20.0% in EV71 severe and other-pathogen severe groups.Capillary refilling time prolongation incidences were 36.1% and 13.3%,indicating a statistically significant difference(p?0.05).The incidences of the piebald and sychnosphygmia did not have any significant difference in both groups(p>0.05).4.Abnormality of the respiratory system did not have any statistically significant difference in both groups(p>0.05).5.The severe EV71 group had higher white blood cell count,higher blood glucose value and higher c-reactive protein value than other severe pathogen groups(P < 0.05);The results of the Creatine Kinade MB,percentage of neutrophils and percentage of lymphocytes are on the opposite(p>0.05).The white blood cells of cerebrospinal fluid had statistically significant difference in both groups(p?0.05)while there did not exist any significant difference in the cerebrospinal fluid protein,glucose and chlorine(p>0.05).6.The age of onset,scattering percentage,exposure history and vomit incidence had statistically significant differences in both severe and mild EV71 severe groups(p?0.05)while the gender did not in both groups.(p>0.05).7.The positive percentage for the Mycoplasma hyopneumoniae antibody detection were separately 48.5% and 18.5% in EV71 severe group and other-pathogen severe group,indicating a statistically significant difference(p?0.05);42.0% and 18.5% were for mild EV71 severe group and other-pathogen severe group,showing a a statistically significant difference(p?0.05)while there were no statistically significant difference between the severe and mild EV71 infected children(p>0.05).Conclusions: 1.EV71 infection is more likely to involve the nervous system and circulatory system than other pathogens.2.The positive rate of mycoplasma pneumoniae was high in EV71 patients with HFMD.
