Objective: To compare the clinical effect of craniotomy versus sphenotresia drainage treatment for hypertensive intracerebral hemorrhage in Blend Sign positive patients.Methods: A total of 200 patients with hypertensive ICH in the basal ganglia were divided into two groups(positive group and negative group)according to the presence or absence of computed tomography(CT)“Blend Sign”.Patients in each group were randomly divided into craniotomy subgroup and puncture drainage subgroup.The rebleeding rate,hospital stays were compared between the two subgroups in the positive and negative groups.Results: For the blend sign positive group,the puncture drainage subgroup there were no significant differences in rebleeding rate(P>0.05),the blend sign positive subgroup had significantly shorter hospital stays than the blend sign positive group(P<0.05).For the blend sign positive group,the craniotomy subgroup there were no significant differences in rebleeding rate and hospital stays(P>0.05).For the blend sign positive group,the craniotomy subgroup had significantly higher hematoma clearance rate than the puncture drainage subgroup(P <0.05),there were no significant differences in rebleeding rate(P>0.05).Conclusion: Compare with craniotomy,puncture drainage is more simple,smaller trauma,more fast,and not increase the rebleeding rate.Treatment for spontaneous intracerebral hemorrhage in blend sign positive patients is safe and reliable,clinical application and dissemination.
