PurposeThrough the investigations of TCM syndrome types of patellofemoral osteoarthritis and the X-ray films of patellofemoral joints, they provide reference for the prevention of patellofemoral osteoarthritis,TCM syndrome differentiation and treatment and accurate diagnosis of X-ray films, they also provide important information for studies related to patellofemoral osteoarthritis.MethodAccording to the selection criteria of case, research objects were included in the study and their basic information and X-ray films of patellofemoral joints were collected.TCM syndrome differentiation was achieved based on information of inquiry of TCM.The composition features of TCM syndrome types of patients with patellofemoral osteoarthritis were statistical analyzed.Observation indicators including:(1)the composition of TCM syndrome types;(2)the proportion of TCM syndrome types of patients with different sides and genders;(3)the features of age, course of disease, and body mass index of patients with different TCM syndrome types;(4)the composition of K-L grade of X-ray films of research objects with different TCM syndrome types.Logining China Telecom medical image cloud system,X-ray films of patellofemoral joints that meet the selection criteria of this study were screened out and their basic information was recorded.included in the study. The types of X-ray films of patellofemoral joints were determined by measuring the related parameter. The features of X-ray films of patellofemoral joints were statistical analyzed, analysis indicators including:(1)The composition of Wiberg's types of patellar; The features of age of patients with different Wiberg types of patellar.(2)The composition of types of patellofemoral dysplasia; The features of age of patients with different types of patellofemoral dysplasia.(3)The composition of X-ray films types of patellofemoral joints; The features of age of patients with different types of X-ray film of patellofemoral joint.(4)The features of age and the course of disease of patients with different K-L grades of X-ray film.ResultsAmong the patients with different TCM syndrome types of patellofemoral osteoarthritis, patients with deficiency of the liver and kidney type account for 35%,patients with stasis-blood-obstruction type,wind-cold-damp impeding type, and wind-damp-heat impeding type account for 27.5%,22.5%,and 15%respectively.The gender ratio of male and female research object is 1:2.08;There is no significant difference in the composition of TCM syndrome types of patients with different genders and sides(P>0.05).There is no statistically difference in the average age of patients with different TCM syndrome types(P>0.05);There is statistically difference in the composition of TCM syndrome types of patients in different age groups(P<0.05);The age of patients with the deficiency of the liver and kidney type,stasis-blood-obstruction type, and wind-damp-heat impeding type are mainly distributed in the 40-59 age group; There is no statistically significant difference in the distribution of patients with wind-cold-damp impeding type among the three age groups. There is no statistically difference in the BMI of patients with different TCM syndrome types(P>0.05).The course of patients with the deficiency of liver and kidney type is longer than that of patients with wind-cold-damp impeding type and wind-damp-heat impeding type(P<0.05).There is no significant difference in the composition of the K-L grade of X-ray films of the patellofemoral joints among different TCM syndrome types(P>0.05).In the Wiberg'type of the patella of 200 patients, the composition ratios of type I patella, type II patella, and type III patella are 13%,65%and 22%respectively,and the composition ratio of type II patella is the highest(P<0.01);The composition ratio of female patients with type I patella, type II patella, and type III patella is higher than that of male(P<0.01);There is no significant difference in everage age of patients with different Wiberg's types of patella(H=0.509,P=0.775).There is no significant difference in the composition of type of femur trochlear dysplasia in different genders(P>0.05);The composition ratio of patients with type B femur trochlear dysplasIa is higher than that of type A(x~2=6.681,P=0.010);There is no significant difference of male and female gender ratio in the femur trochlear dysplasia of type A,B and C(P>0.05);The average age of patients with type A femur trochlear dysplasia is the smallest(P<0.01).Among various types of X-ray films of patellfemoral joint, the composition ratios of patients with patella alta, small patella, and femoral trochlear dysplasia are 26%,22%and 20.5%respectively and there is no statistical difference among the three types(P>0.05);The composition ratios of patients with patellar maltracking, lateral deviation, and the complex of small patella with patella alta are 13.5%,11.5%and 6.5%respectively and there is no statistical difference among the three types(P>0.05);The composition ratios of female patients with patella alta, small patella, femoral trochlear dysplasia, and patellar maltracking are higher than that of male(P<0.05);There is no significant difference of gender ratio of male and female patients with lateral deviation and the complex of small patella with patella alta(P>0.05);There is no statistically significant difference in the average age of patients with different types X-ray plain films of patellofemoral joints(P>0.05).There is a positive correlation between the K-L grade of the X-ray films of the patellofemoral joint and the three age groups, and the correlation is weak; The average age of the research objects with K-L grade III is the largest(P<0.01),followed by grade II(P<0.01),and the average age of grade I is the smallest(P<0.01).There is a moderate correlation between the K-L grade of X-ray films of the patellofemoral joint and the course of disease; The average course of disease is the longest in patients with K-L grade III of X-ray films of the patellofemoral joints(P<0.01),followed by grade II(P<0.01),and the average course of disease of grade I is the shortest(P<0.01).ConclusionIn the common TCM syndrome types of patellofemoral osteoarthritis, the deficiency of the liver and kidney type is the most, and the wind-damp-heat impeding type is relatively rare; There is no significant correlation between the K-L grades of X-ray films of patellofemoral joints and TCM syndrome types. The composition ratio of patients with type II patella is highest among all the patients with patellofemoral osteoarthritis; The composition ratio of patients with type B femur trochlear dysplasia is higher than that of type A.In all types of patellfemoral joint X-ray films, the patella alta, small patella, and femoral trochlear dysplasia are common. The composition ratios of female patients with patella alta, small patella, femoral trochlear dysplasia, and patellar maltracking are higher than that of male; There is a positive correlation between the severity of X-ray films of patellofemoral joint and the age of patients with patellofemoral osteoarthritis, and the correlation is weak. The severity of the X-ray films of patellofemoral joint was positively correlated with the course of patellofemoral osteoarthritis.
