The Analysis Of Curative Effect On The Correction Of High Ametropic Amblyopia With Rigid Gas Permeable Contact Lenses

OBJECTIVE: To research the efficacy,stability,security and changing regularity of correcting vision of rigid gas permeable contact lenses(RGPCL)for patients with high ametropic amblyopia.METHODS: To observe patients(8 years old to 16 years old)91 cases(123 eyes)willing to wear RGPCL in combination with the comprehensive treatment of amblyopia,who came to the ophthalmologic clinic room in Lanzhou Second Hosipital as their best corrected visual acuity(BCVA)can not be improved continuously by spectacles combine with comprehensive treatment of amblyopia for more than six months and were diagnosed with high ametropic amblyopia by attending physicians or above from March 2015 to March 2017.We chose optimum lenses for patients according to each patient’s optometry results and keratometry preoperatively,meanwhile,the changes of BCVA,comfortableness and security were observed at 1 week,1month,3 months,6 months and 1 year postoperatively.Based on the types of high refractive error,the patients were divided into groups for stratification and integrated studies.One-way ANOVA,Chi-Square test,Fisher probabilities in data,LSD-t test were used to analyze the data.RESULTS:(1)Compared with wearing spectacles in combination with the comprehensive treatment of amblyopia,the treatment of RGPCL in combination with the comprehensive treatment of amblyopia improved the BCVA at 1 week,1 month,3 months,6 months and 1 year,and the difference was statistically significant(P<0.05).(2)After RGPCL treatment in combination with the comprehensive treatment of amblyopia for 1 week,more than fifty percent of eyes achieved an increasing BCVA of 2 lines or more(E Standard Logarithm Eyesight Table).(3)The degree of amblyopia in the group of RGPCL treament in combination with the comprehensive treatment of amblyopia was lower than that in the group of spectacles treatment in combination with the comprehensive treatment of amblyopia at 6 months postoperatively(except for super-high myopia with low to moderate astigmatism or simple super-high myopia group),and the difference was statistically significant(P<0.05).(4)There is no statistical difference in BCVA among RGPCL treatment in combination with the comprehensive treatment of amblyopia at 1month,3 months,6 months and 1 year and 1 week(P>0.05).(5)On the first day of RGPCLwearing,the main symptoms were as follows: 115 eyes(93.50%)with dry eyes or mild foreign body sensation,and 29 eyes(23.57%)with itching.The discomfort of those symptoms disappeared after one month.In the first week of RGPCL wearing,the cornea epithelial cells were partly detached in 5 eyes(4.07%),and the symptom disappeared after one month.No severe complications such as keratoconjunctival infection,corneal neovascularization,giant papillary conjunctivitis,etc.occurred during follow-up.CONCLUSION:(1)Compared with spectacles in combination with the comprehensive treatment of amblyopia,RGPCL in combination with the comprehensive treatment of amblyopia can improve the BCVA of patients with high refractive error and mild to moderate degree of amblyopia from 8 to 16 years old.Meanwhile,the effect was stable and the treatment is safe.(2)Compared with spectacles in combination with the comprehensive treatment of amblyopia,patients wearing RGPCL in combination with the comprehensive treatment of amblyopia for 6months had a more decrease in degree of amblyopia.(3)The BCVA was improved most rapidly in RGPCL treatment in combination with the comprehensive treatment of amblyopia for 1 week,and the effective rate of amblyopia treatment was more than 50%,after which the visual correction effect remained stable.
