Objective Using multiple inversion time-pulsed arterial spin labeling(m TI-ASL)and intravoxel incoherent motion imaging(IVIM)to observe the regional cerebral blood perfusion in patients with T2 DM,and then to explore the application value of them in the early stage of diabetic brain damage.Methods 34 objects(11 control objects and 23 patients diagnosed as type 2 diabetes mellitus by clinicians)were scanned by Siemens Magnetom Skyra 3.0T MRI.The scan sequences included routine sagittal T1 weighted image,axial T1 weighted image,axial T2 weighted image,T2-tirm,DWI(b=0 s/mm2,1000 s/mm2),m TI-ASL and IVIM.m TI-ASL was performed using echo planer imaging(EPI)sequence with 4 inversion time of 1500,1990,2480,2970 ms.IVIM was performed using a prototype single shot echo planer imaging(SS-EPI)sequence with 10 b values of 0,50,100,150,200,300,400,600,800,1000s/mm2.The regions of interest(ROIs)were drawn on the white matter of bilateral frontal lobe,temporal lobe and parietal-occipital lobe respectively in the maps of CBF,ATT and IVIM derived from image post-processing softwares.Values for cerebral blood flow(CBF),arterial transit time(ATT),f,D*and D were read and recorded.The parameters of different regions between control group and T2 DM group were analyzed using independent sample t test.The correlation analysis between m TI-ASL and IVIM selected Pearson correlation test.Results1.Comparison of demographic characteristics between control and T2 DM groups The blood glucose of control group was(5.3±0.7)mmol/L,the blood glucose of T2 DM group was(7.5±2.6)mmol/L.It was obvious that the blood glucose level of T2 DM group was higher(P<0.05)than control group.The systolic blood pressure value of control group was(121.3±10.5)mm Hg,the systolic blood pressure value of T2 DM group was(141.1±12.3)mm Hg.T2 DM group had higher systolic blood pressure value(P<0.05)than control group.Other indicators such as age,BMI and blood lipid were not statistically different between control and T2 DM groups.2.Comparison of related parameters of m TI-ASL at different regions between control group and T2 DM group The CBF values of white matter on frontal lobe,temporal lobe and parietal-occipital lobe were(2.15±0.25)ml·100g-1·min-1,(2.44±0.28)ml·100g-1·min-1 and(1.69±0.28)ml·100g-1·min-1 respectively in control group.The CBF values of white matter on frontal lobe,temporal lobe and parietal-occipital lobe were(1.78±0.19)ml·100g-1·min-1,(2.13±0.35)ml·100g-1·min-1 and(1.72±0.26)ml·100g-1·min-1 respectively in T2 DM group.Compared with control group,the CBF values of white matter on frontal and temporal lobe in T2 DM group were smaller(P<0.05).The ATT values of white matter on frontal lobe,temporal lobe and parietal-occipital lobe were(0.15±0.01)ms,(0.14±0.02)ms and(0.14±0.03)ms respectively in control group.The ATT values of white matter on frontal lobe,temporal lobe and parietal-occipital lobe were(0.17±0.02)ms,(0.16±0.02)ms and(0.14±0.02)ms respectively in T2 DM group.The ATT values of white matter on frontal and temporal lobe in T2 DM group were longer than in control group(P<0.05).The values of CBF and ATT in the white matter of parietal-occipital lobe were not different between control and T2 DM groups.3.Comparison of related parameters of IVIM at different regions between control group and T2 DM group In control group,the values of f,D*and D in the white matter of frontal lobe were(0.03±0.00)%,(0.02±0.01)x10-3mm2/s and(0.00±0.00)x10-3mm2/s respectively.In T2 DM group,the values of f,D*and D in the white matter of frontal lobe were(0.02±0.01)%,(0.02±0.01)x10-3mm2/s and(0.00±0.00)x10-3mm2/s respectively.The control group had larger f and D*values(P<0.05)in the white matter of frontal lobe than T2 DM group,the value of D was not different between the two groups.In control group,the values of f,D*and D in the white matter of temporal lobe were(0.02±0.01)%,(0.02±0.00)x10-3mm2/s and(0.00±0.00)x10-3mm2/s respectively.In T2 DM group,the values of f,D*and D in the white matter of temporal lobe were(0.01±0.00)%,(0.02±0.00)x10-3mm2/s and(0.00±0.00)x10-3mm2/s respectively.The control group had larger f and D*values(P<0.05)in the white matter of temporal lobe than T2 DM group,the value of D was not different between the two groups.In control group,the values of f,D*and D in the white matter of parietal-occipital lobe were(0.02±0.00)%,(0.01±0.00)x10-3mm2/s and(0.00±0.00)x10-3mm2/s respectively.In T2 DM group,the values of f,D*and D in the white matter of parietal-occipital lobe were(0.02±0.01)%,(0.02±0.00)x10-3mm2/s and(0.00±0.00)x10-3mm2/s respectively.The values of f,D* and D in the white matter of parietal-occipital lobe between the two groups had no obvious difference.4.The correlation analysis between m TI-ASL and IVIMThere was no correlation between CBF in m TI-ASL and f ?D* in IVIM.The f was significantly positively related to D*(r?=0.837**,r?=0.885**)?Conclusion1.Both m TI-ASL and IVIM can detect the changes of regional cerebral blood perfusion in patients with T2 DM and have certain application value in the early stage of diabetic brain damage;2.There is no significant correlation between the perfusion parameters of m TI-ASL and IVIM;3.Compared with control group,the blood perfusion of frontal and temporal lobe in T2 DM group decrease.
