Pectus excavatum is a kind of thoracic deformity in adolescents with high incidence and the Nuss minimally invasive orthopedic surgery as the representative of deformity chest orthopedics has been widely used in the clinical treatment of funnel chest.The operation began to be used in China after entering the 21 st century.But the mechanical environment of the postoperative chest bone is seldom studied.Its reconstruction and calculation methods are complex,which is difficult to analyze large sample cases.There is a lack of the study on the effect of intercostal muscle on the correction of pectus excavatum with scoliosis at the time of minimally invasive correction.Also it has not been known the impact of intercostal muscles on both sides on the correction effect of scoliosis.The author's research team is studying the intercostal muscles on the effect of the pectus excavatum with scoliosis by zoopery and numerical simulation.In experimental study,The author uses the slaughtered sheep and makes experimental model of the thoracic with scoliosis.The paper studied the intercostal muscles on both sides of socliosis and different height position of intercostal muscles on the effect of thoracic deformation and scoliosis through loading experiment of animal thoracic model.In the aspect of numerical simulation,the paper developed a three-dimensional simplified solid model and a hybrid calculation model which was a combination of solid element and beam element on the basis of the existing research work.Based on these two models,we create a fine model of pectus excavatum and scoliosis with the intercostal muscles.The fine model of deformity thorax model with intercostal muscles successfully solved the problem that a deformed thoracic assembly model created by 3D reconstruction can not meet the construction of the fine model and rules of the intercostal muscles.The numerical simulation and parameter study show that intercostal muscles have obvious influence on the deformation of deformity thorax.The restriction effect of collapsing side intercostal muscles in the process of sternum elevation is obvious,which shows pulling force.The effect of normal side intercostal muscles is relatively weak.The effect of computing model considering intercostal muscles in scoliosis deformation is that orthopedic results of scoliosis are improved.But the intercostal muscles hindered the scoliosis deformation improvement.The result is in agreement with the clinical results.Intercostal muscles can delay the scoliosis aggravating effect of deformation for the contralateral and lateral superior deformity of the chest.The shape and state by mechanical regulation in the growth of bones are changed.There are many factors that need to be observed and studied in the process of minimally invasive orthopedics and reconstruction of pectus excavatum.At present,only very few literature reports pectus excavatum minimally invasive orthopedic postoperative bone reconstruction process of shaping research at home and abroad.There is no related research on the shaping process of the ribs after the operation and thoracic springback problem after removing orthopedic plate.The author carried out the carrier experiment of SD rats and made preparations for designing the special orthopedic instruments in rats and determining the factors influencing the shaping process.Through the animal experiment of Nuss orthopedic surgery after the shaping process,we can study the effect of bone reconstruction on the plastic process after thoracic orthopedics,predict the morphology and density distribution of the chest bone,provide the basis for the design of the operation plan and improve the curative effect.
