| Objective:1.To understand the status of cognitive function in patients with laryngeal carcinoma,explore whether patients with laryngeal carcinoma have cancer-related cognitive impairment,at the same time,understand which area of cognitive decline occur and provide reference for selecting evaluation tools in the next work.2.To analyze the difference in cognitive function between patients with laryngeal carcinoma and healthy volunteers of the same age,sex,educational level similar to provide a reference for the construction of nursing program for patients with laryngeal cancer-related cognitive impairment and preliminary results for the study of clinical cancer-related cognitive impairment.3.According to previous studies,the nursing plan for patients with laryngeal cancer-related cognitive impairment was constructed to provide a reference for the care of patients with cancer-related cognitive impairment,at the same time provide practical guidance for the further development of nursing interventions related to laryngeal cancer-related cognitive impairment.Methods:1.Qualitative interviews were performed in patients who have been first diagnosed with laryngeal carcinoma,aware of their condition but not received any treatment by using a self-made interview plan.2.Taking the first diagnosed patients who come from a Department of Otorhinolaryngology Head and Neck Surgery in two Three Class A Hospitals in a Province with laryngeal carcinoma and healthy volunteers of the same age,sex,educational level similar as the research object,Wechsler Intelligence Scale for Adut-Chinese Revised(WAIS-RC)and Wechsler Memory Scale(WMS)were used to assess the cognitive status of the study subjects.3.Based on Orem's self-care theory and Nursing Interventions Classification,combined with qualitative and quantitative methods to establish the initial construction of nursing plan for patients with laryngeal cancer-related cognitive impairments.To study the established nursing plan with Delphi method twice,collate and summarize the experts' opinions and suggestions to establish the final nursing plan for patients with laryngeal cancer-related cognitive impairment.Results:1.In a semi-structured interview of nine patients with laryngeal carcinoma,it was found that all of the interviewees had poor memory and attention.Seven interviewees thought it was related to the disease but the other two interviewees believed it was only the normal phenomenon of human aging.2.No significant difference of age,sex and educational level was found between both groups(P>0.05).The scores of verbal intelligence quotient(VIQ),performance intelligence quotient(PIQ),full intelligence quotient(FIQ)and memory quotient(MQ)were significantly lower in the laryngeal carcinoma group than healthy control group(P<0.05);the picture arrangement,information,similarities,block design,picture completion,digit symbol of WAIS-RC between two groups have significant(P<0.05),and there was a significant difference in the results of WMS except experience,orientation and association test(P<0.05).3.Two rounds of expert consultation,respectively,of an analysis of the consulting results,the laryngeal cancer-related cognitive impairment patients nursing program shows which is constituted by three entry.The Kendall's W of these three entries in the first round of expert consultations respectively were 0.350,0.248,0.30.The Kendall's W of these three entries in the second round of expert consultations respectively were 0.378,0.308,0.380.The results all between 0 and 1 showed a small increase in the trend,meanwhile the difference was statistically significant(P<0.01).The Delphi's research process is more scientific and feasible,and the findings have a high reliability.Conclusion:1.Patients with laryngeal carcinoma complaint about cancer-related cognitive impairment and have greater needs for nursing knowledge related to cognitive ability.They hope to acquire the knowledge of cognitive function based on nursing education meanwhile coexist with other methods.2.laryngeal carcinoma disease group was not as Memory(Long-term,Short-term,Immediate memory),Attention(eg,Figure recognition,Association thinking),Visuospatial memory(eg,Block design),Executive functioning(eg,Block design,Picture arrangement),Processing speed(eg,Block design,Digit symbol),Abstract epitome ability(eg,Similarities),Think logically(eg,Picture arrangement)as the healthy control group.3.Finally,a nursing program for patients with laryngeal cancer-related cognitive impairment including 4 first-level entries,12 second-level entries and 57 third-level entries was formed.After two rounds of Delphi expert consultation,the nursing program of patients with laryngeal cancer-related cognitive impairment can better meet patients' with laryngeal carcinoma demands.It has high scientificity and reliability,in the mean while provides scientific basis for nursing measures for cancer-related cognitive impairment patients. |
