Objective:To explore the effect of astragalus injection and the possible mechanism on myocardial injury in young sepsis rats by detecting the change of c TnI in peripheral blood serum and IL-6,IL-10 in myocardial tissue and the expression of protein JAK2 and protein STAT3..Methods:1.A total of 48 specific pathogen free Sprague-Dawley rats of 5 week years old were randomly divided into 3 groups : Sham-operated group,CLP group,astragalus injection group,and each group was randomly divided into 8h and 24 h time points(n=8).The model of sepsis pups rats was established by using cecal ligation and perforation.The Sham group find the cecum after opening the laparotomy just only stiring the cecum without other treatment.The CLP group find the cecum after opening the laparotomy,ligature and perforate in the middle of cecum.Then inject normal saline(5ml/Kg)into the enterocoelia immediately.The AS group surgical procedure AS CLP group,and inject the same dose of astragalus injection into the enterocoelia immediately.After the operation,each rat were injected normal saline(30ml/Kg)for fluid resuscitation to supplement the accumulated loss.2.Each group are collected blood from abdominal aorta and gather myocardial tissue at 8h or 24 h time points after surgery.To detect the change of cTnI in peripheral blood serum and IL-6,IL-10 in myocardial tissue by enzyme-linked immunosorbent assay.To observe the expression of P-JAK2 and P-STAT3 in myocardial tissue by immunohistochemistry.To observe the pathological changes of myocardial tissue by hematoxylin-eosin staining.Results:1.The pathological changes of myocardial tissue: The Sham group myocardial cells and fibers morphology and histology are without obvious abnormalities;Compared with Sham group,the CLP group morphology and histology have some pathological changes: myocardial cells are edema with vacuolar degeneration,some nuclear pyrolysis,and the myocardial fibers arrangement are loosing and disordered.Compared with CLP group,the AS group two time points,the myocardial cells and fibers morphology and histology are improved.2.The level of cTnI in serum changes: Compared with Sham group each time point,the level of cTnI in serum in CLP group and AS group increased significantly(P <0.01);Compared with CLP group,the AS group serum the cTnI concentration decreased obviously at 8h time point(P <0.05)and decrease markedly at 24 h time point(P <0.01);Compared with 8h time point in CLP group and ASgroup,the level of cTnI in serum at the 24 h time point increased obviously((P <0.05)),while in the Sham group between two time point is no significant difference(P=0.586).3.The changes of myocardial tissue IL-6: Compared with Sham group each time point,the level of myocardial tissue IL-6 in CLP group and AS group increased markedly(P <0.01);Compared with CLP group each time point,the IL-6 concentration in AS group decreased markedly(P <0.01);Compared with 8h time point in CLP group and AS group,the IL-6 concentration at the 24 h time point increased obviously(P <0.05),while in the Sham group between two time point is no significant difference(P=0.509).4.The changes of myocardial tissue IL-10: Compared with Sham group each time point,the level of myocardial tissue IL-10 in CLP group and AS group increased obviously(P <0.05);Compared with CLP group each time point,the IL-10 concentration in the AS group is no changes at 8h time point(P=0.318),but decrease markedly at 24 h time point(P <0.01);Compared with 8h time point in CLP group and AS group,the concentration at the 24 h time point is increased obviously,while in the Sham group between two time point is no significant difference(P=0.485).5.The expressions of myocardial tissue JAK2 protein: Compared with Sham group each time point,the JAK2 protein expressions in CLP group and AS group increased markedly(P<0.01);Compared with CLP group,the AS group concentration decreased obviously at 8h time point(P <0.05)and decrease markedly at 24 h time point(P <0.01);Compared with 8h time point in CLP group and AS group,the expressions of myocardial tissue JAK2 protein at 24 h time point is increased markedly(P <0.01),while in Sham group between two time point is no significant difference(P=0.558).6.The expressions of myocardial tissue STAT3 protein : Compared with Sham group each time point,the expressions of STAT3 protein in CLP group and AS group increased markedly(P <0.01);Compared with CLP group each time point,expressions of myocardial tissue STAT3 protein in AS group decreased markedly(P <0.01);Compared with 8h time point in CLP group and AS group,the expressions at 24 h time point increased obviously,while in the Sham group two time poin is no significant difference(P=0.599).7.The correlation analysis between the expression of myocardial tissueJAK2 protein and the level of myocardial tissue IL-6 and IL-10 : the expression of myocardial tissue JAK2 protein is positively correlated with IL-6 and IL-10 at 8h or 24 h time point in CLP group(r>0.7,P <0.05).8.The correlation analysis between the expression of myocardial tissue STAT3 protein and the level of myocardial tissue IL-6 and IL-10 : the expression of myocardial tissue STAT3 protein is positively correlated with IL-6 and IL-10 at 8h or 24 h time point in CLP group(r>0.7,P <0.05).Conclusion:Astragalus injection plays a protective effect on myocardial injury of sepsis pups rats.And the possible mechanism is by interferencing the activation of JAK/STAT signal transduction pathway to reduce the release of pro-inflammatory IL-6 and increase the release of anti-inflammatory IL-10,and then improving the damage of pro-inflammatory/ anti-inflammatory disequilibrium in myocardial cells.
