PURPOSE:In this experiment,rabbit osteonecrosis of the femoral head caused by the use of large doses of hormones,and then treated with Gushiling Capsule,analysis of changes in serum lipid metabolites and histopathological and radiological.To investigate the early changes in lipid metabolism characteristics of Gushiling Capsule on steroid-associated necrosis of the femoral head(SAFNH)and its mechanisms for treating SAFNH.METHODS:100 healthy adult Japanese rabbits were selected,all males,blood serum lipids were analyzed by blood collection from the ear veins,and experimental rabbits with abnormal biochemical abnormalities were excluded.Randomly divided into blank control group,model group,Guexiling capsule group,lovastatin group.Except for the blank control group,large doses of glucocorticoid(10 mg/kg)were used to construct a femoral head necrosis model.After the injection of hormones,each group was given a daily gavage treatment.Goutialin capsule group was given 15 ml of Goutienling capsule suspension,lovastatin group was given lovastatin suspension 15 ml,and control group and model group were given 15 ml distilled water.Each group was continuously gavage for 4 weeks.At the 2nd and 4th week after treatment,four groups of sera were collected.At the same time,3 rabbits from each group were randomly selected for testing,and four groups of high-density lipoprotein(HDL-c)and total cholesterol(TC)and triglyceride(TG)and low density lipoprotein(LDL-c)were measured.HE staining were used to observe the histopathological changes of the femoral head in each group.MRI image analysis technique was used to analyze the morphological changes of femoral head in each group.RESULTS:(1)Serum test results: Biochemical test results showed that compared with the control group at the 2nd week,the serum lipids in the other three groups showed significant metabolic disorders,TC,TG,and LDL levels increased significantly,and HDL levels decreased,and the differences were statistically significant.Significance(P<0.01);At the 4th week,compared with the model group,the levels of TC,TG,and LDL in the gushiling capsule group and the lovastatin group were significantly lower,and the difference was statistically significant(p<0.01).There was no significant difference between gushiling capsule group and lovastatin group.(2)HE staining results: The blank control group had normal morphology,few empty bone lacunae,and thick and complete trabeculae.No disorder was observed.The morphology of bone cells was intact.No eosinophil aggregation and adipocyte hyperplasia were observed.In the model group,trabecular deformation was observed.The accumulation of fat droplets and the accumulation of eosinophils were observed in the bone marrow cavity.The more bone cells were atrophied and the empty bone lacunae increased.The bone trabecula shape and structure of gushiling capsule group were improved,empty lacunae increased,and a small amount of eosinophils were seen in the bone marrow cavity and a smoll amount of adipocyte hyperplasia was observed.In the lovastatin group,the trabecular shape and structure of the trabecula were improved,and the empty lacunae were increased.A small amount of adipocyte hyperplasia and a small amount of eosinophils were observed in the bone marrow cavity.(3)In the control group,the bilateral femoral heads had smooth surfaces,regular patterns,no abnormal signal changes inside the femoral head,and bilateral hip joint space was normal.In the model group,the femoral head zygomatic line was blurred on both sides,the surface of the femoral head was uneven,the shape was not regular,and irregular long T1 and short T2 change regions appeared inside the femoral head.T2 WI suggested bilateral hip space enlargement and fluid accumulation in the joint cavity,and bilateral interstitial fluid in soft tissue increased.There was no abnormal signal band under the head of the femoral head in the gushiling capsule group,and the joint space was normal.The effusion of the soft tissue in the cavity and surrounding area was reduced.In the lovastatin group,bilateral femoral head zygomatic lines were slightly blurred,no abnormal signal was changed under the head,and the joint space was normal.The effusion in the intraluminal and surrounding soft group was reduced.CONCLUSION:Lipid metabolism disorder is closely related to hormone-induced avascular necrosis of the femoral head.Chinese medicine gushiling capsule can prevent and relieve the progression of femoral head necrosis by regulating the body's lipid metabolism early and lowering serum lipid levels.
