A Follow-up Analysis On 68 Patients With Ileocecal Inflammation

Objective:To investigate the clinical features and natural course of ileocecal inflammation on 68 patients,providing guidance to clinical treatment based on the regularity concluded from different outcomes of these patients.Methods: 68 patients with ileocecal inflammation at Xinjiang Medical Hospital in the past 7years have been retrospectively collected and analyzed for the clinical manifestations,laboratory examinations,endoscopic features and pathological features.These patients had follow-up visits regularly for their health condition and final outcomes.Results:68 cases of ileocecal inflammatory lesions were visited for an average of 84 months,23 cases(33.8%)recovered without special treatment,their preliminary visit of colonoscopy shows that they only had the terminal ileum single congestive erosion.17 cases(25.0%)were diagnosed with intestinal tuberculosis.The symptoms of fever and night sweats were common at the time of first visit.The CPR and ESR were significantly increased,HB decreased obviously,and the colonoscopy often showed multiple irregular ulcers.8cases(11.8%)were diagnosed with inflammatory bowel disease,among which 3 cases were Crohn’s disease and 5 cases were ulcerative colitis.these patients were often accompanied symptoms of weight loss,CPR and ESR were common in newly diagnosed cases.The positive rate of OB test high,colonoscopy often manifested as multiple longitudinal ulceration along the intestinal wall.Conclusion:Inflammatory lesions of the ileocecal caused by multiple factors of which clinical diagnosis is difficult,Its diagnosis depends on the diagnostic treatment and long-term follow-up.In this study,it was also found that most of the patients with single congestion and erosion of the ileal terminal were benign lesions,and it was less likely to develop into CD or TB.The high incidence of tuberculosis in the region,The patients with the symptoms of low fever night sweats,ESR,CRP increased,HB decreased,colonoscopy involving both the cecum and terminal ileumshould arouse vigilance for the possibility of the future development of intestinal tuberculosis.At present,TB and CD are still difficult to identify,It is necessary to perform diagnostic anti-TB treatment.The clinical symptoms should be closely followed up while the diagnostic anti-TB treatment is performed.The healing of ulcer under endoscope should observed at the same time,evaluating the therapeutic effect and changing the treatment strategy in time.
