Objective To explore the status of mitochondrial functional in the different viscera deficiency COPD patients and the effect to COPD patients disease severity by a cross-sectional study.Methods 161 patients with stable COPD were enrolled.Recording history and according to the deficiency of Zang Fu the standard discriminate the TCM syndromes.Pulmonary ventilation function was measured,evaluation of CAT and m MRC,a six minute walk test(6MWT)was performed.The nucleated cells were isolated from peripheral blood of patients,and mitochondrial membrane potential and intracellular reactive oxygen species(ROS)were detected by flow cytometry.SPASS21.0 was used to analyze the data by one-way ANOVA and correlation analysis.Results In 161 cases of COPD,36 cases were diagnosed as lung qi deficiency,31 cases of lung and spleen qi deficiency,50 cases of lung and kidney qi deficiency,44 cases of lung,spleen and kidney deficiency,of which 104 cases completed the mitochondrial experiment.The course of disease in COPD patients was the longest in the lung and spleen qi deficiency group,and the shortest in the lung qi deficiency group,and there was no significant difference between the 4 groups(P=0.503).The pulmonary ventilation function FVC% was the lowest in the lung and spleen qi deficiency group and the highest in the lung qi deficiency group,but there was no significant difference between the 4 groups(P=0.265).FEV1%had also significantly different among groups(P=0.025).The COPD assessment test(CAT)had significantly different among groups(P=0.001).There was no significant difference in6 MWD between the 4 groups(P=0.057),but there was significant difference between thelung qi deficiency group and the lung,spleen,lung,spleen and kidney deficiency group(P<0.05).The mean values of FEV1%,FVC% and 6MWT were lowest in the lung,spleen and kidney deficiency group,and the mean of CAT was the highest in the lung,spleen and kidney deficiency group.The mitochondrial membrane potential(MTP)in the lung and spleen deficiency group and Qi deficiency group,lung and kidney qi deficiency group had significant difference(P<0.05);intracellular reactive oxygen species in the lung and spleen deficiency group and Qi deficiency group,lung and kidney qi deficiency group had significant difference(P<0.01).There was no significant difference in the mitochondrial membrane potential and intracellular reactive oxygen species between the Qi deficiency group and the lung qi deficiency group.The mitochondrial membrane potential decreased significantly in the lung spleen qi deficiency group,and the reactive oxygen species increased significantly in the lung spleen qi deficiency group;the mitochondrial membrane potential was correlated with the reactive oxygen species(r=0.223,P=0.021).Conclusion 1.As the degree of asthenia of viscera becomes more and more serious,the condition of COPD becomes more and more serious,and the COPD with the characteristics of lung,spleen and kidney three viscera deficiency is the most serious.The prevention and treatment of COPD should pay attention to the role of deficiency of Zang Fu in the pathogenesis of COPD.2.Mitochondrial dysfunction is associated with spleen deficiency in patients with COPD,suggesting that invigorating spleen therapy may improve mitochondrial dysfunction.
