The Clinical Observation On The Treatment Of Rheumatoid Arthritis With The Acupuncture Of "guben Tongluo"

Objective:It observed the clinical effects of "Guben Tongluo" +methotrexate(MTX)in the treatment of patients as well as the symptom with rheumatoid arthritis(RA),the clinical effects of C-reactive protein(CRP),rheumatoid factor(RF),erythrocyte sedimentation rate(ESR)through the regulating effects during a series of laboratory indicators.At the same time,observing the above-mentioned method takes effective in liver dysfunction,the alleviation of the digestive tract reaction and Leukopenia that caused by MTX.The clinical efficacy of patients with a comprehensive and objective evaluation provides a scientific basis for the treatment of RA,which is beneficial for clinical promotion.Methods: From March 2016 to March 2017,50 patients,who were qualified as RA diagnostic criteria,were selected from rheumatoid arthritis ward in Wuhan Hospital of TCM.They were randomly divided into treatment group and control group according to admission sequence.The control group was given MTX10 mg orally once a week.The treatment group was combined with acupuncture treatment on the basis of the control group.Acupuncture acupoint selections were shown to you as follows.On the one hand,we selected Guanyuan,Qihai,Tianshu,Zhongwanacupoints as the main point.On the other hand,we selected adjunct acupuncture points in a general way,such as local acupoints,followed by meridians.Local acupoints:the ones with shoulder joint disease select Jianyu,Jianliao and so on;the ones with elbow joint disease select Quchi,Chize and so on.Followed by meridians:for those with the painful part of the meridian syndromes,the ying-spring point and shu-spring point of the meridian should be selected.For instance,the Large Intestine Meridian of Hand-Yangming select Erjian,Sanjian and Triple energizer meridian select Zhongzhu,Yemen and so on.It was required to examine 3 times a week,1 month for a course of treatment,and the whole treatment period took 3 courses in totally.This study should pay highly attention to observing the changes of those two group before and after during the whole experiment.Several changes of laboratory indexes such as DAS-28 score,symptom-index score,and laboratory indexes of RF,ESR,CRP should be taken consideration.At the same time,studying on alanine aminotransferase(ALT),gastrointestinal adverse reactions and white blood cell(WBC)was a good way for us to get familiar with its safety.Results:(1)Comparison of symptom and index score: Comparing the two results,it is obvious to show that there were significant differences in symptom and symptom index points between the treatment group and the control group before and after treatment(P<0.01).In addition,the effect of the treatment group was better than the control group,which took effect in symptoms and signs remission through the experimentperiod(P<0.05).(2)DAS-28 score comparison:Through the study achievement,the scores of DAS-28 before and after treatment had a big difference in those two groups.And those two groups were treatment group and the control group(P<0.01).What's more,the effect of improving DAS-28 score in treatment group was superior to the control group(P<0.05).(3)Laboratory indicators comparison: Before the treatment,the various laborary index appeared no significant difference between the treatment groups and the control groups.After the treatment,the indexes of RF,ESR,CRP all decreased extremely sharply in both groups(P<0.01).Moreover,the treatment group was advantage over the methotrexate group in lowering the initial laboratory index(P<0.05).As the result,the treatment group was especially effective in reduction of CRP(P<0.01).(4)Adverse reactions comparison:There were cases of adverse reactions of digestive tract in treatment group.While we could find that 8 cases of adverse reactions of digestive tract appeared in the control group.The adverse reactions of digestive tract in two groups were 4.17% and 32% respectively(P<0.05).Moreover,after 30 days and 90 days in the treatment,it was easy to recognize that the final experimental result of the treatment groups was much better than the control groups for improving liver function(P<0.05,P<0.01).At the same time,"Guben Tongluo" acupuncture therapy improved myelosuppression better than the control group(P<0.05).(5)Comprehensive efficacy comparison:Among the 24 patients in the treatment group,the experimental data showed that 6patients were cured totally,16 patients' health conditions had improved and only 2 patients had no improvement on health after the treatment.The total effective rate was 91.67% in the treatment group.For the control group,there were 25 patients had been observed.Among the 25 patients in the control group,4 patients were markedly effectively cured,16 patients' health conditions had improved and the rest of 5 patients were ineffective in the end.We could make the conclusion that the total effective rate was 80% in the control group.After comparing these two groups,there was no significant difference between the treatment group and the control group(P> 0.05).In conclusion : "Guben Tongluo" + methotrexate group and methotrexate group in the treatment of RA have achieved significant clinical efficacy.Patients in those two groups both get improvement to some extent in the following aspects,such as symptom and signs index scores,DAS-28 scores and others.While comparing the control group,the treatment group is superior in above indexes."Guben Tongluo" + methotrexate treatment group is beneficial to relief the symptom.It is also good at reducing side effects of pharmaceutical drugs when controlling the disease.In addition,this treatment method helps to strengthen the clinical efficacy.Thus,"Guben Tongluo" + methotrexate treatment is one of the ideal methods of RA treatment,which is valuable for clinical promotion.
