Objective:By comparing the changes of TNF-α,hs-CRP,IL-6,UA,Scr and BUN in peripheral blood of patients with gout attack after coronary intervention,exploring the mechanism and the correlation of related inflammatory factors and PCI.Method:Enrolled in patients with previous gout or hyperuricemia history who need coronary intervention treatment as the object of study from October 2015 to december 2017,collecting 472 cases of which stent implantation in 108 cases,simple angiography in 364 cases.The patients with previous tumors,tissue damage,liver and kidney dysfunction,infection and those diseases which can causes inflammation changes are excluded.The diuretics and aspirin are avoiding.Record the situation of postoperative acute attack of gout according to inflammatory reaction.Record personal information such as age,sex,body mass index(BMI),hypertension,diabetes,hyperlipidemia,and smoking history.The criterion is the acute attack of gout within 3 days after operation.Cases was divided into the attack group and the non-attack group.Cases was also divided into PCI group and CAG group according to whether the stent is implanted.The peripheral blood samples of all patients were collected one day before operation and one day after operation.The levels of serum inflammatory factors were detected,mainly including TNF-α.hs-CRP、IL-6、UA、Scr and BUN.Result:A total of 472 patients were involved,including 53 gout attacks(11.23%).PCI group incloud 108 patients and 19(17.59%)of gout attacks.CAG group incloud 108 patients and 34(9.34%)of gout attacks.And the difference was statistically significant(P<0.05).Before the operation,attack group and non-attack group have no statistically difference at the levels of hs-CRP、IL-6、TNF-α、UA、Scr and BUN(P>0.05).After the operation,comparing in the group:attack group patients hs-CRP,IL-6,UA levels postoperative higher than preoperative,the difference is statistically significant(P<0.05).The non-attack group patients hs-CRP、IL-6、TNF-α、UA、Scr and BUN levels postoperative and preoperative has no obvious change,there was no statistically difference(P>0.05).The attack group patients TNF-α、Scr、BUN levels postoperative and preoperative has no obvious change,there was no statistically difference(P>0.05).Comparing between the group:the attack group patients hs-CRP、IL-6、UA levels postoperative higher than non-attack group,the difference is statistically significant(P<0.05).The two group patients TNF-α、Scr、BUN levels have no statistically difference(P>0.05).Conclusion:1.In interventional therapy,inflammatory factors such as hs-CRP and IL-6 may be related to the mechanism of acute attack of gout.2.The changes of hs-CRP,IL-6 and UA in the blood after operation can predict the gout attack early after PCI or CAG.3.High-dose contrast agent more likely to cause acute exacerbation of postoperative gout.
