Clinical Effect Of Total Hip Arthroplasty In Patients With Osteoporosis

China’s aging population is more and more serious,people with osteoporosis increases,including femoral neck fracture,femoral head necrosis,rheumatoid arthritis is common in elderly patients with osteoporosis,total hip arthroplasty(total hip,arthroplasty,THA)can improve their hip joint function and life quality.As a common surgical curative effect of the majority of patients received satisfactory.ObjectiveTo observe the effect of THA on patients with osteoporosis throughout the day and to understand the necessity of postoperative anti osteoporosis treatment.MethodThe object of study a total of 129 people were selected from July 2014 to March 2017 in the First Affiliated Hospital of Chengdu Medical College and the parallel THA patients,a retrospective case-control study was performed,including 71 women,58 men,the minimum age of 67 years,the maximum age of 87 years old,the average age of 75 years.Of 134,128 were single hips and 6 were double hips,with a total of 140 hips.The patients with hip fracture in 75 cases(42 cases on the right side,33 cases on the left side),50 cases of aseptic necrosis of femoral head(30 cases on the right,20 cases on the left side),9 cases of rheumatoid arthritis(6 cases on the right side,3 cases on the eft side)over 3 cases,prosthesis repair(2 caseson the right side of the left side in 1 cases),3 cases of hip osteoarthritis(all left).Among all 134 patients,preoperative complications were 73(56.59%),comorbidities including diabetes,hypertension,chronic obstructive pulmonary disease,pulmonary infection,coronary heart disease,cerebral infarction,bedsore,chronic renal failure,hypoproteinemia,thrombocytopenia,hypoproteinemia,anemia,electrolyte disorder.There were 56 patients(43.41%)without obvious complications,26 patients(20.16%)had one complication before operation,33 patients(25.58%)had two kinds of complications,14 patients(10.85%)had three or more than three kinds of complications.All patients were treated with the modified Gibson approach,using the mixed bone cement(femoral lateral acetabular biotype,bone cement)artificial hip replacement,long-term follow-up after THA,and suggested that all patients with anti osteoporosis treatment after surgery(129 patients,treated with anti osteoporosis treatment 82 the remaining 47 people who refused for anti osteoporosis treatment),rehabilitation after the operation were recorded and statistics of patients before and after 3,6 and December hip function score(Harris score),and health survey scores(the medical outcomes study item short from health survey,SF-36)were evaluated in patients before and after 3、6、12 months changes in the level of quality of life.ResultAll 134 patients with THA included 128 cases of single hips,6 cases of double hips,140 cases of hips,3 cases of fat liquefaction due to hip wounds,healing by second intention,and the rest of wounds healing by first intention,and no wounds were found in all patients.The longest follow-up duration of 46 months,the shortest 12 months,an average of 26 months.During the follow-up of 3 patients out of contact,2 patients died after discharged,one died 3 months after operation,another one died 7 months after operation.In 129 patients,received postoperative anti osteoporosis treatment are 82,the remaining 47 patients refused to accept anti osteoporosis treatment,anti osteoporosis treatment of patients without prosthesis loosening and periprosthetic fractures,2 patients had thigh pain,47 patients refused for anti osteoporosis treatment in,10 patients had complications,2 of them appeared thigh pain,3 had femoral prosthesis loosening and sinking,2 patients had dislocation,2 patients with deep venous thrombosis,1 patients with periprosthetic fracture.The average score of Harris was 15.19 points before operation,50.60,81.48 and 92.19 at 3 months,6 months and 12 months after operation.The excellent and good rate of hip joint score was 58.91%,84.50% and 93.02% respectively after 3 months,6 months and 12 months.The treatment of anti osteoporosis is less than that of no anti osteoporosis treatment.All patients with postoperative complications,significantly improve the level of quality of life all day.ConclusionThe overall efficacy of THA in patients with osteoporosis is satisfactory,Mixed bone cement prosthesis is applied to the elderly patients with osteoporosis after operation and the good rate of hip joint function is high.Obviously improving the quality of life of the patients,the treatment of anti osteoporosis after operation can reduce the incidence of complications.
