ObjectiveThree dimensional speckle tracking imaging(3D-STI)was applied to evaluate the changes of left ventricular global systolic function in children with acute viral myocarditis before and after treatment,so that it can provide a theoretical basis for clinical diagnosis and treatment of the disease.MethodsThirty-eight children with viral myocarditis were screened as the observation group and 38 healthy children as the control group.The observation group was divided into the pre-treatment group and the post-treatment group.The blood biochemical tests,routine ultrasonic cardiogram,and 3D-STI techniques were used to measure and record data from the observation group before and after the treatment and the control group.The relevant parameters were analyzed and compared: myocardial troponin I(cTnI)in the myocardial enzymes,amino-terminal B-type natriuretic peptide(NT-proBNP),left ventricular end-diastolic diameter(LVEDd),left ventricular end-systolic diameter(LVEDs),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),left ventricular ejection fraction(LVEF),left ventricular global longitudinal strain(LVGLS),left ventricular global radial strain(LVGRS),left ventricular global circumferential strain(LVGCS),left ventricular global area strain(LVGAS),and left ventricular wall strain parameters at difference stages.Results1.Comparison of measured parameters of conventional echocardiographyThere was no significant difference in the measurement parameters LVEDd,LVEDs,LVEDV,LVESV and LVEF between the observation group and the control group before and after treatment,compared with the control group(P>0.05).2.Comparison of the results of three dimensional strain parameters(1)The longitudinal strain,radial strain,circumferential strain and area strain of left ventricle in the observation group were significantly different from those in the control group and the observation group after treatment(P<0.05).(2)Longitudinal strain: the strain values of left ventricular segments in the observation group were lower than those in the normal control group after treatment,and the difference was statistically significant(P<0.05).The segment of anterior wall of anterior wall,middle segment,apical and lateral wall basal segment,middle segment and apical segment(P<0.01)were more obvious segments.(3)Radial strain: the observation group before the treatment of left ventricular myocardial radial strain and after treatment and compared to normal controls except the inferior basal segment,middle segment of inferior wall and posterior wall of the middle and inferior apical segment,residual myocardial strain of each segment were decreased,the difference was there statistical significance(P<0.05).(4)Circumferential strain: the strain values of the left ventricular segments in the observation group were lower than those in the normal control group after treatment,and the difference was statistically significant(P<0.05).The segment of the anterior wall of the anterior wall,the middle segment,the apical segment and the lateral wall,and the middle segment(P<0.01)were more obvious.(5)The area strain compared with the observation group before thetreatment of left ventricular myocardium strain area and after treatment and normal control group,the strain values were decreased,the differences were statistically significant(P<0.05),and the decrease is more obvious with anterior segment of basal and middle segments and apical and basal lateral wall segment and the middle segment(P<0.01).Conclusions1.The acute heart of the children with viral myocarditis can show the overall and local systolic function of the left ventricular myocardium.2.Three-dimensional speckle tracking imaging can sensitively reflect the changes of left ventricular systolic function before and after treatment,so that it can provide a good theoretical basis for clinical and scientific research.
