Objective:To investigate the distribution regular pattern of TCM syndromes of chronic atrophic gastritis in climacteric women and to analyze the correlation between the age,mental and psychological factors,dietary bias,pathological results of gastric mucosa,Hp infection,KI scale and other factors,so as toprovide some theoretical reference for clinical diagnosis and treatment.Methods:From January 2016 to December 2017,a total of 100 women who were diagnosed as CAG in the general internal medicine,digestive department,endoscopy center and ward of the Jiangsu Province Traditional Chinese Medicine Hospital were diagnosed and diagnosed by gastroscopy.A total of 100 cases were collected,and the clinical observation table of Chinese medicine for chronic atrophic gastritis in menopause and the KI scale were filled in.The general situation of the patients,the symptoms and signs,the mucosal condition under the gastroscope,the pathology,the Hp infection and the TCM syndromes were recorded,and the statistical analysis was carried out by the statistical software SPSS19.0Results:1.In this study,age 40-49 is defined as a low age group,and 50-60 is defined as a high age group.In the age distribution,there were 38 patients in the low age group,62 patients in the high age group,and the ratio of the two groups is 1:1.63,indicating that the incidence of the high age group was higher than the lower age group,suggesting that the age of the age group was closely related to the incidence of CAG among menopausal women.2.Menopausal women with low age group CAG were mainly characterized by mild atrophy of the gastric mucosa.The proportion of moderate to severe atrophy in the high age group was higher.It indicates that age is closely related to the degree of gastric mucosal atrophy.The age is older,the higher rate of moderate to severe atrophy.3.The incidence of intestinal metaplasia in the low age group was 92.1%and the pathological changes were mild,the incidence of intestinal metaplasia in the high age group was 89.2%and proportion of moderate intestinal metaplasia was higher,suggesting that the age of CAG in climacteric women was not related to gastric mucosa intestinal metaplasia but was related to the degree of intestinal metaplasia.The older the age was,the greater the degree of intestinal metaplasia.In this statistics,there is no severe intestinal metaplasia.4.The overall dysplasia in gastric mucosa was 10%.The incidence in the low age group was 13.2%and in the high age group,it was 8.1%and mild in the dysplasia.After examination,P>0.05,indicating that age was not related to gastric dysplasia and its degree.5.The infection rate of Hp in the low age group was 39.5%and the infection rate of Hp in the high age group was 32.3%.After examination,P>0.05,indicating that there was no correlation between age and Hp infection.6.The liver-qi stagnation syndrome was the most common of distribution of TCM syndromes of CAG patients in menopause.The distribution of TCM syndromes from high to low was liver-qi stagnation syndrome,stagnated heat in liver and stomach syndrome,deficiency of stomach yin syndrome,deficiency cold of spleen and stomach syndrome,spleen-stomach damp-heat syndrome,and stomach blood-stasis syndrome7.The KI scale and mental and psychological factors are closely related to the distribution of TCM syndromes.The KI score of liver-qi stagnation syndrome is usually low,with mild climacteric symptoms,while the KI score of stagnated heat in liver and stomach syndrome,and spleen-stomach damp-heat syndrome is higher and climacteric symptoms are heavier.8.The distribution pattern of TCM syndromes is closely related to Hp infection and bile reflux.The Hp infection rate of spleen-stomach damp-heat syndrome is the highest,up to 66.7%and the Hp infection rate of deficiency cold of spleen and stomach syndrome is the lowest,which is 10%.The total incidence of bile reflux was 24%,of which the incidence of stagnated heat in liver and stomach syndrome was the highest,reaching 50%.9.The distribution pattern of CAG in climacteric women was not correlated with age and eating habits.10.Climacteric women with CAG are mostly accompanied by mental and emotional abnormalities,such as mental stress,anxiety,depression,impatience and irritability.Mental and psychological factors,dietary factors and climatic factors are the main causes of CAG in climacteric women.Among them,mental and psychological factors induce the highest rate of liver-qi stagnation syndrome and the ratio of climate factors to deficiency cold of spleen and stomach syndrome is the highest.11.Most atrophy of the gastric mucosa in climacteric women with CAG was accompanied by the pathological changes of intestinal metaplasia and a few of the pathological changes of dysplasia.There was no correlation between the distribution pattern of syndromes and intestinal metaplasia and dysplasia.Conclusions:The most common syndrome of CAG in climacteric women is liver-qi stagnation syndrome,frequently combined with the disequilibrium between gallbladder and stomach,and the stagnation of liver-qi and deficiency of the spleen.It has been summarized that the stagnation of liver-qi and stomach-qi is the therapeutic approach in the clinical treatment.Meanwhile,the clearing of gallbladder and protecting spleen-earth should be focused on.We also should pay attention to emotional illness,emphasize the change of sentiment and character,as well as the improvement of mood of patients and have a regular daily life.
