Study On Reimbursement Effect Of Health Insurance Schemes For Inpatient Families In Rural China In The View Of Health Expenditure Poverty

[Purpose]This study uses CHE and IHE to analyze the status of health expenditure poverty of inpatients family in rural areas,in order to understanding the reimbursement effect of the rural medical security system,and provide policy suggestions for improving the security level of inpatients in rural areas,to alleviate health expenditure poverty Effectively.[Methods]We introduce domestic and foreign related theories and research status of health expenditure poverty,arid draw on some research methods.Through agency survey,to get the basic operation data of NCMS,serious illness insurance,medical assistance and the data of NCMS inpatient cost of M County from 2014 to 2015.And,health seeking behavior and medical expenses of 536 Inpatients are collected by household survey.On this basis,use Documentary Research Method,Statistical Research Method and Counterfactual Research Method,,to analyze the relevant data and information.[Results]The incidence of CHE after medical insurance compensation was 24.25%,19.96%,22.01%,and 19.78%,using the income method,expenditure method,non-survival expenditure method,and WHO method.Among them,income method is most sensitive to low-income people.The expenditure method,non-survival expenditure method,and WHO method are sensitive to low-income and high-income people both.The capacity of rural health insurance system of alleviating CHE is limited.And nearly 50%families which has CHE are not in the scope of health insurance.High-income people also facing the uncertaintiessuch as disease risk.The incidence of IHE measured by the relative proportion method,Engel's coefficient method,and survival need method was 5.60%,12.87%,and 7.28%.The IHE distributed in low-income groups mainly.Poor households and poverty-stricken family households have higher poverty rates due to health expenditure.[Conclusions]As the main methods of measuring health expenditure poverty,CHE and IHE can measure the economic burden and the rate of IFOOP from the perspective of relative costs and absolute costs.The two indicators fully consider the family's ability to pay and can effectively define the health expenditure poverty families which fall into poverty because the health expenditure exceed households ability to pay.Reasonable measurement of household payment capacity is the key of defining the object accurately.Local governments need to formulate appropriate standards according to specific conditions.To a certain extent,the medical insurance system alleviated the health expenditure poverty of inpatients in rural areas.But the effect is insufficient.Medical assistance should be the focus of the medical system construction content.
