Objective:To evaluate the clinical values of the susceptibility weighted imaging(SWI)in meningioma.Methods:Fifty patients with pathologically confirmed WHO Grade?meningiomas were enrolled into this study between October 2015 and February 2018.All cases underwent the SWI and MRI scans preoperatively.Imaging features,including degree of edema,intratumoral hemorrhage,calcification and drainage vein,were statistically analyzed by Mann-Whitney U test or Chi-Square test,accompanying with pathological Ki67 and progesterone receptor index.Results:The incidence of microhemorrhages detected by the SWI in meningiomas was 14%,which were scattered and dotted-like.The Edema index(EI),maximum edema width and Ki67 in the microhemorrhagic group were all significant higher than that of the non-microhemorrhagic group(3.35±2.34 vs 2.06±2.05,U=62.00,P<0.05;3.67±1.83cm vs 1.48±1.95cm,U=53.00,P<0.01;8.86%±5.43%vs 4.23%±1.85%,U=47.50,P<0.01,respectively).Thirty-one calcifications in meningiomas were detected by CT;and SWI found 47"calcifications",including 16"micro-calcifications"that CT did not show.Diameters of"micro-calcifications"were less than 1mm.Eleven meningiomas(61.11%)clearly showed dominant draining veins originating from the tumor itself among the 18 cases without peritumoral edema;the other 5 meningiomas(15.63%)forming efferent venous drainage from the tumor were observed in the 32 meningiomas with peritumoral edema(?~2=10.95,P<0.01).Conclusions:SWI had advantages for brain tumors'microhemorrhage,calcification and vein,thus providing a useful complement to the conventional MRI sequence.Intratumoral hemorrhages infrequently occurred in WHO Grade?meningioma.Microhemorrhagic meningioma had higher Ki67 and more serious peritumoral edema than the non-hemorrhagic one.Tumor-related veins can be detected by the SWI,which helped identifying and protecting them during operations and studying the mechanism of peritumoral edema formation in meningioma.
