PurposeTo analyze DWI?diffusion weighted imaging DWI?performances of normal cervical and cervical cancer,To Explore the value of ADC?apparent diffusion coefficient ADC?which derived from DWI?diffusion-weighted imaging DWI?in the diagnosis?the pathological feature and degree of cervical cancer,and predicting the response of cervical cancer to concurrent chemotherapy.MethodExperimental group of cervical:Conventional MR and DWI?b=1000 s/mm2?were performed in 20 patients without uterine disease.The signal intensities of the normal cervical were observed and the ADC values of them were measured.The group of cervical cancer:The signal intensities of 150 patients pathologically diagnosed as cervical cancer were observed and the ADC values of them were measured,The mean ADC values of cervical cancer and normal cervical were compared;The correlation was analyzed between ADC and the pathological feature and degree of cervical cancer.Fourty-three patients with advanced cervical cancer underwent conventional MRI and DWI examinations before therapy and at two-three weeks after the initiation of concurrent chemotherapy and after the treatment completly done,The longest diameter of tumor before and after treatment was measured,The ADC was calculated,compared and correlated with final tumour retracton.Results1.Normal cervical in the T1WI showed equal signal;Normal cervical mucus,inner zone,combined and packed four-layer structure in T2WI showed high signal,medium signal,low signal,medium signal;Normal cervical endometrial?junctional zone and myome-trium three-layer structure ADC values were?1.36±0.16?×10-33 mm2/s??1.54±0.14?×10/-33 mm2/s??2.01±0.23?×10-33 mm2/s,Combined signal and ADC values were lower than the rest of the two-tier,structure,the mean ADC value?1.64±0.18?×10-33 mm2/s.2.Most cervical cancer lesions in the T1WI showed equal signal,showed slightly higher signal on T2WI,All tumors of cervical cancer demonstrate high signal intensity,the mean ADC value of cervical cancer lesion is?0.94±0.13?×10-33 mm2/s,cervical cancer ADC values were significantly lower than the normal cervix average ADC value,the difference was statistically significant?P<0.05?;There were statistic differences between squamous carcinoma?0.89±0.11?×10-33 mm2/s and adenocarcinoa?1.04±0.12?×10-33 mm2/s of the cervial cancer,Squamous carcinoma of Cervical cancer has different degrees of differentiation ADC value:high-differention group?1.01±0.11?×10-33 mm2/s?middle-differen-tiated?0.87±0.12?×10-33 mm2/s?poorly-differentiated group?0.68±0.14?×10-33 mm2/s,There is negative correlation between ADC value quamous carcinoma of Cervical cancer and the grades of tumor?r=-0.676,P=0.008?.3.The ADC value of cervical cancer after chemotherapy were increased,The ADC values of prior to chemotherapy?2-3 weeks and after the chemotherapy was?0.91±0.13?×10-3 mm2/s??1.37±0.14?×10-3 mm2/s??1.53±0.22?×10-33 mm2/s,it showed significant difference each other?P<0.01?.The change of ADC between prior to chemotherapy?2-3weeks therapy was?0.43±0.12?×10-33 mm2/s,final tumor retraction was?0.80±0.11?,NO correlation was foud between ADC values and tumour,while the changes in ADC showed a significand correlation with tumour retraction?P<0.05?.Conclusion1.DWI-MRI has a potential ability to differentiate between Normal cervical and cervical cancer,The ADC can be used to indicate the pathological feature and degree of cervical cancer,which will be an ieal tool to evaluate the pathological feature and degree of cervical cancer.2.The ADC values of cervical cancer increased after chemotherapy,the early change of ADC value showed a correlation with final tumour retraction,which will be an ieal tool to predicte the response of cervical cancer to concurrent chemotherapy.
