bjective: To compare the therapeutic effects of injectable small intestinal submucosa and film-like small intestinal submucosa in repairing of cartilage defects in a rabbit knee,evaluating the effect of injectable small intestinal submucosa.Methods : 27 adult New Zealand white rabbits(weighing 3.0-3.5kg)were used in this study,osteochondral defects(4.0 mm in diameter,3.0mm in depth)were created in the femoral trochlea of the patellofemoral joint of bilateral knees,then the rabbits were randomly divided into three groups,the first group was blank group,the second group was film-like small intestinal submucosa(SIS)treated group,the third group was injectable small intestinal submucosa treated group.The rabbits were euthanized at 4,8 and 12 weeks after surgery,and gross observation,HE staining,toluidine blue staining,type ? collagen immunohistochemical were made,and the ICRS standard was used to scoring the gross observation and histological grading,the results of type?collagen immunohistochemical staining at 12 weeks were analyzed by Image-pro Plus 6.0 image analysis software,the content of glycosaminoglycans(GAG)in each group at 12 weeks was measured by spectrophotometer colorimetric method.Results:(1)Gross observation: 4 weeks after surgery no new organization was found in the blank group,while in the control group and experimental group a few of gray organization can be found.8 weeks after surgery new organization was found in each group,the surface was rough,the boundary with normal tissue was clear.12 weeks after surgery the gray organization in each group was more than that in 8 weeks,in the blank group and control group the boundary with normal tissue was clear,while in the experimental group the boundary with normal tissue was fuzzy.(2)Gross grading of ICRS: 4 weeks after surgery were 1.33±0.52,2.33±0.52,3.50±0.55;8 weeks after surgery were 2.17±0.75,4.67±0.82,7.33±0.82;12 weeks after surgery were 7.83±0.75,10.17±0.75,11.17±0.98.4 weeks and 8 weeks after surgery there were significant difference among the three groups(P<0.05);12 weeks after surgery there was no significant difference between experimental group and control group(P>0.05),but compared with the blank group were significant difference(P<0.05).(3)Histological observation: HE staining,toluidine blue staining,type ?collagen immunohistochemical showed 4 weeks after surgery there was no cartilage repaired in each groups and the extracellular matrix stains was negative.8 weeks after surgery much of fibrous can be found in the blank group and the extracellular matrix stains was negative,while in the control group and experimental group the extracellular matrix stains was weak positive.12 weeks after surgery fibrocartilage can be found in the blank group,the boundary with normal tissue was fuzzy,the extracellular matrix stains was negative.While in the control group and experimental group hyaline-like cartilage can be found and the extracellular matrix stains was positive.(4)ICRS histological scores: 4 weeks after surgery were 3.67±0.52,5.50±0.54,6.33±1.03;8 weeks after surgery were 5.50±0.84?5.83±0.75?7.67±0.82;12 weeks after surgery were 6.83±0.75?7.00±0.63?10.33±1.03.4 weeks after surgery there was no significant difference between experimental group and control group(P>0.05),but compared with the blank group were significant difference(P<0.05).8 weeks and 12 weeks after surgery there was no significant difference between blank group and control group(P>0.05),but compared with the experimental group were significant difference(P<0.05).(5)The levels of mean density of type ?collagen immunohistochemical staining in each group at 12 weeks were 0.2183 ± 0.05672,0.3333 ± 0.04033,0.4833 ± 0.06121,and there were significant difference among the three groups(P<0.05).(6)The levels of glycosaminoglycan(GAG)in each group at 12 weeks were 8.46±0.92?g/mg,11.17±0.63?g/mg,13.15±1.01?g/mg,and there were significant difference among the three groups(P<0.05).Conclusions: Both film-like SIS and injectable SIS can promote the repair of rabbit knee articular cartilage defects,but the injectable SIS is better.
