Objective:To study the effect of neck circumference on left ventricular remodeling and left ventricular diastolic function in hypertension patients with phlegm-blood stasis syndrome,to provide new insights into the prevention and treatment of hypertension.Research Methods:Using the database from patients with phlegm-blood stasis syndrome at outpatient department and cardiology department and people undertaking health check in The Fujian University of Traditional Chinese Medicine Hospital.Among the selected 280 patients who met the inclusion and exclusion criteria.The values of the neck circumference were measured,and according to the median of the neck circumference,the patients were divided into groups:healthy male with low neck circumference,healthy male with high neck circumference,healthy female with low neck circumference,healthy female with high neck circumference group,hypertensive male with low neck circumference,hypertensive male with high neck circumference,hypertensive female with low neck circumference,and hypertensive female with high neck circumference.Left ventricular structure and diastolic function index of the selected patients were measured by echocardiography.The proportion of normal configuration group,centripetal hypertrophy group,centripetal reconstruction group and centrifugal hypertrophy group were counted.Statistical analysis of data.Result:1.Healthy male,healthy female,hypertensive male,and hypertensive female were divided into high and low neck circumference with 38cm,34cm,39cm and 35cm respectively.2.The differences in LVDd,LVDs,IVST,PWT,LVM,RWT and LVMI between healthy male with low neck circumference group and healthy male with high neck circumference group have no statistical significance(P>0.05).The differences in the normal configuration,centripetal remodeling,centripetal hypertrophy and centrifugal hypertrophy between these two groups have no statistical significance(P>0.05).3.The differences in LVDd,LVDs,IVST,PWT,LVM,RWT and LVMI between healthy female with low neck circumference group and healthy female with high neck circumference group have no statistical significance(P>0.05).The differences in the normal configuration,centripetal remodeling,centripetal hypertrophy and centrifugal hypertrophy between these two groups have no statistical significance(P>0.05).4.The values of LVDd,PWT,LVDs,IVST,LVM and LVMI in the hypertension male with high neck circumference group were significantly higher than those in the hypertension male with low neck circumference group,and the difference was statistically significant(P<0.05).The normal configuration of the hypertension male with high neck circumference group was significantly different from that of the hypertension male with high neck circumference group(P<0.05).The proportion of patients with centripetal hypertrophy and centrifugal hypertrophy in hypertension male with high neck circumference group was significantly higher than that in hypertension male with low neck circumference group.5.The values of PWT?LVDs?IVST?LVM?LVMIin the hypertension female with high neck circumference group were significantly higher than those in the hypertension female with low neck circumference group,and the difference was statistically significant(P<0.05).The normal configuration of the hypertension female with high neck circumference group was significantly different from that of the hypertension female with high neck circumference group(P<0.05).The proportion of centripetal hypertrophy and centrifugal hypertrophy in hypertension female with high neck circumference group was significantly higher than that in hypertension female with low neck circumference group.6.The differences in LVSF,LVEF,E wave,A wave,IVRT,DT,S wave,D wave,Pva,VP,Em,Am,E/A,Em/Am between healthy male with low neck circumference group and healthy male with high neck circumference group have no statistical significance(P>0.05).7.The differences in LVSF,LVEF,E wave,A wave,IVRT,DT,S wave,D wave,Pva,VP,Em/Am.E/A between healthy female with low neck circumference group and healthy female with high neck circumference group have no statistical significance(P>0.05)8.The value of IVRT and DT in the hypertension male with high neck circumference group was significantly higher than that in the hypertension male low neck circumference group,and the difference was statistically significant(P<0.05).The numerical value of E/A and D wave in the hypertension male with high neck circumference group was significantly lower than that in the hypertension male low neck circumference group,and the difference was statistically significant(P<0.05).9.The numerical value of IVRT in the hypertension female with high neck circumference group was significantly higher than that in the hypertension female low neck circumference group,and the difference was statistically significant(P<0.05).The numerical values of S wave and E/A in the hypertension female with high neck circumference group were significantly lower than those in the hypertension female with low neck circumference group,and the differences were statistically significant(P<0.05).Conclusion:1.High neck circumference has no effect on left ventricular remodeling and left ventricular diastolic function in healthy men and women.2.High neck circumference has negative effect on left ventricular remodeling in male and female patients with phlegm-blood stasis syndrome,it is mainly reflected in the increase in the proportion of patients with cardiac hypertrophy and centrifugal hypertrophy.3.High neck circumference worsens left ventricular diastolic dysfunction in male and female hypertension patients with phlegm-blood stasis syndrome.
