Objective Contrast Super microvascular(SMI)with Color Doppler flow imaging(CDFI)in the diagnostic sensitivity of the blood flowing in breast neoplasm.And joint BI-RADS classification criteria for the diagnosis of breast neoplasm in order to make it clear whether SMI and mammography X-ray combined application can improve the efficiency of the diagnosis of breast neoplasm.Methods This study was a retrospective analysis of 82 patients who received treatment and hospitalization in the first Affiliated Hospital of Jinzhou Medical College because of breast diseases during July 2016 to July 2017(a total of 88lesions).Prior to the implementation of routine pathological diagnosis were ultrasound and mammography X-ray photography.According to Adler grading criteria,observe the differences in blood flowing between CDFI and SMI.At the same time evaluate them by BI-RADS classification diagnostic criteria In combination with histopathological analysis to analyse the value of SMI imaging in breast masses.Compare SMI?mammography X-ray and the combined to get their sensitivity?specificity and accuracy in the diagnosis of the breast masses.At last determine the optimal diagnostic method.Result According to Adler grading standard,CDFI and SMI were used to observe the blood flowing of benign malignant tumor in breast.Detecting rate of blood flowing with CDFI was 79.55%(70/88)and with SMI was 85.23%(75/88).Different Adler grades in CDFI and SMI had statistically significant difference(P=0.017,P=0.02).The results of blood flowing distribution were shown: The blood flowing around the benign and malignant masses by CDFI was statistically significant(P=0.006),but there was no statistically significant difference in the center and penetrating blood flowing(P=0.157,P=0.207).There was statistically significant difference in the center and penetrating blood flowing of the benign and malignant masses by SMI(P=0.005,P=0.004),but there was no statistically significant difference in around blood flowing(P=0.108).With pathological diagnosis as the gold standard,the corresponding sensitivity,specificity,accuracy of SMI combined with BI-RADS classification were87.50%,91.67%,89.77% and the corresponding sensitivity,specificity,accuracy of CDFI combined with BI-RADS classification were82.50%,89.58%,87.50%.According to ROC curve,it shows that the area under curve,standard error and 95% confidence interval of SMI-BI-RADS were higher than those of CDFI-BI-RADS(0.883vs0.848;0.040vs0.045;0.804-0.962 vs 0.759-0.936),but the difference was not statistically significant(Z=0.618,P=0.536).The sensitivity,specificity,accuracy of X-ray mammography BI-RADS classification in the diagnosis of breast masses were 77.50%,87.50%,82.95%.The sensitivity,specificity,accuracy of SMI-BI-RADS joint X-ray mammography in the diagnosis of breast masses were 95.00%,93.75%,94.31%.According to ROC curve,it shows that the area under curve,standard error and 95%confidence interval of SMI-BI-RADS joint X-ray mammography were higher than those of SMI-BI-RADS and X-ray mammography(0.944 vs0.883 vs0.825;0.029vs0.040 vs 0.048;0.888 – 1 vs0.804-0.962 vs0.731-0.919).SMI-BI-RADS joint X-ray mammography help improve the sensitivity,specificity,accuracy of clinical diagnosis breast masses.The difference between SMI-BI-RADS and SMI-BI-RADS joint X-ray mammography was not statistically significant(Z=1.013,P=0.310).The difference between X-ray mammography and SMI-BI-RADS joint X-ray mammography was statistically significant(Z=2.408,P=0.016).Conclusions Compared with CDFI,SMI is more sensitive in displaying microvascular in breast masses.Mammography X-ray show higher rates of calcification inside the breast neoplasm.Combining SMI and mammography X-ray to judge benign and malignant breast neoplasm has certain clinical value.
