Under the strategic goal of building a "healthy China," although China's basic medical security services have already achieved "universal health insurance," the issue that people are most concerned about is "difficulty in seeing a doctor and expensive to see a doctor" has not been fundamentally solved,and therefore China's medical insurance system faces more arduous challenges and tasks.Under such a background,the state has paid more attention to the core issues of medical security and the problem of inadequate protection for critical illness and has rapidly carried out and promoted major illness insurance work nationwide.At present,the financing method for critical illness insurance is allocated from the urban and rural residents' medical insurance funds according to a certain percentage or quota.Under the hope of the state's desire to quickly achieve full coverage of critical illness insurance,critical illness insurance will put pressure on the income and expenditure gap of the rural and urban residents' medical insurance funds.If it is drastically consume the balance of existing urban and rural residents' medical insurance funds to achieve full coverage of critical illness insurance,not only will the major illness insurance system be difficult to sustain,but the health care system of the urban and rural residents that it depends on will also be greatly affected.In order to provide valuable policy recommendations for the critical illness insurance system,it is necessary to study the important factors affecting the incidence of critical illness.The level of economic and social development between Yunnan Province and other provinces in the country and even within Yunnan Province is quite different.The spatial and temporal differences and the influencing factors of the incidence of critical diseases in Yunnan Province are representative.Most of the researches in China and abroad mainly analyze the ways,operating mechanisms and emerging problems of urban and rural residents' critical illness insurance in the perspective of macroeconomic policies.All these studies are based on the perspectives of policy makers,and few are based on the incidence of critical illness analysis.Based on the micro-data of workers in urban and rural areas in medical mutual aid of Yunnan Province,this paper analyzes the temporal and spatial disparities of the incidence of critical diseases in Yunnan Province by using the econometric methods based on the relevant macroeconomic indicators and climate and geographic information.By establishing a statistical model,the key factors affecting the incidence of critical illness were analyzed.This article chooses a new perspective to carry on the research,with the hope to provide the valuable policy suggestion for perfecting Yunnan Province critical illness insurance system.In this paper,spatial autocorrelation analysis of the incidence of critical illness in 16 states in Yunnan Province shows that there is spatial correlation between the incidence of critical illness in neighboring cities.According to the time dimension,the analysis of space-time difference shows that from 2012 to 2016,the changes of agglomeration vary with time,but the degree of change is not significant.In general,the spatial agglomeration of the incidence of critical illness is as follows: Most of the areas with high-high concentrations are located in western Yunnan and southwestern Yunnan,while those with low-high concentrations are mostly distributed in western Yunnan and northwestern Yunnan;Mostly distributed in central Yunnan,northeast Yunnan,southern Yunnan.Spatial and temporal variability analysis of the incidence of critical illness in 129 districts and counties in 2012-2016 showed that there was a positive spatial autocorrelation between the incidences of critical illness in all districts and counties in Yunnan Province and there was some change in the spatial aggregation with the passage of time.In a word,areas with high-high and low-low concentrations are more abundant than those with low-high and high-low concentrations.In order to explore the reasons for the spatiotemporal difference in the incidence of critical illness in districts and counties,the explanation variable is the incidence of critical illness in districts and counties.The explanatory variables are as follows: the logarithm of per capita GDP,the logarithm of savings per capita,the annual average temperature,the annual precipitation,the number of beds per million medical institutions.The results of SCAD model showed that: the economic development level,climate and medical level of districts and counties all have an impact on the incidence of critical illness in districts and counties,but the main impact factors are economic development level and climate environment.The main factors that ultimately determine the incidence of critical illness are: per capita savings,annual average temperature and per capita GDP.
