Objective:To observe the effect of Sancao Decoction on acute kidney injury patients with renal tubular injury induced by snake hemotoxins.Changes of clinical symptoms,serum Creatine,PT,APTT,FIB,PLT,CK,BUN,eGFR,and urine NGAL,NAG were evaluated before and after treatment with Sancao Decoction.Effects of tubule function and coagulation function were detected to explore the possible protective mechanism of Sancao Decoction.Methods:92 patients with acute kid injury induced by snake hemotoxins who met the inclusion criteria were enrolled,and randomly divided into control group and Sancao Decoction group.The control group were treated with conventional western medicine,and the Sancao Decoction group were treated with San Cao Decoction(Bergaria Biflora 60 g,Plantain 30 g,Rhizoma Imperatae 30 g,Chinese Herbal Medicine,twice a day)combined with conventional western medicine.Course of treatment was 2 weeks.Before and after the treatment period,the patients' serum Creatine,PT,APTT,Fib,PLT,CK,BUN,eGFR,urine NGAL and NAG were detected respectively.And observe and record the changes of clinical symptoms before and after treatment in each group.Results and data were analyzed by SPSS.Results:1.General data before treatment(gender,age,AKI stage,snake bite site,viper species,blood glucose,blood pressure),observing index(sCr,APTT,PT,PLT,FIB,CK,eGFR,BUN,Urinary NAG,urinary NGAL),safety indexes,TCM symptoms scores,were not statistically different(P>0.05)between two groups and were comparable.2.After treatment,Scr,BUN,urinary NGAL and urinary NAG decreased in both groups,and eGFR increased.The difference between the two groups was statistically significant(P<0.01).Sancao Decoction group was superior to the control group(P<0.01).3.The incidence of proteinuria,hematuria,and edema in both groups decreased after treatment,and there was a statistically significant difference between the two groups(P<0.05).The Sancao Decoction group was superior to the control group.4.After treatment,the APTT decreased in both groups,and the FIB increased.The difference between the two groups was statistically significant.Sancao Decoction group was superior to the control group(P<0.05 and P<0.01).5.After the treatment,the scores of TCM syndromes and clinical composite scores of the two groups of patients decreased significantly.The difference between the two groups was statistically significant.Sancao Decoction group was significantly better than the control group(P<0.05 and P<0.01).6.The clinical comprehensive curative effect of San Cao Decoction group and control group were 93.33%and 84.09%respectively,and the difference was statistically significant(P<0.05).Conclusions:1.Sancao Decoction combined with western medicine treatment may alleviated the renal tubular injury induced by snake hemotoxins,and promote the recovery of renal function,and Sancao Decoction can also reduce proteinuria and hematuria caused by snake hemotoxins.2.Sancao Decoction combined with western medicine can improve the coagulation function of patients with by snake hemotoxins and fire and poisonous syndrome,and promote the recovery of coagulation system.3,San Cao Decoction combined with Western medicine can improve the TCM symptoms and clinical outcomes,and curative effect was better than treatment with Western medicine alone.4.Sancao Decoction had no significant difference in improving liver function,blood leukocyte elevation,PT,PLT rather than.conventional Western medicine treatment.5.No obvious adverse reactions were found during the treatment period of this study in both groups and SanCao Decoction is beneficial for patients with renal tubular injury induced by snake hemotoxins.
