Quadruple Therapy Of Traditional Chinese Medicine Syndrome Differentiation For Treatment Of Acute Pancreatitis And Its Effect On The Serum TNF-?.IL-6.IL-10

Objective: To evaluate the clinical curative effect of quadruple therapy of traditional Chinese medicine syndrome differentiation for treatment of acute pancreatitis(AP)and its effect on the serum tumor necrosis factor-?(TNF-?),interleukin-6(IL-6),interleukin-10(IL-10),to further to explore the mechanism of quadruple therapy of traditional Chinese medicine syndrome differentiation for treatment of acute pancreatitis,and provide a theoretical basis for the research of Chinese medicine treatment of acute pancreatitis.Methods: Acute pancreatitis patients who admitted to the First Affiliated Hospital of Henan University of Traditional Chinese Medicine digestive department from January 2016 to March 2017 were collected,was randomly divided into the treatment group and the control group of 45 cases as the observation object.The control group performed routine treatment of western medicine treatment,fasting,inhibition of pancreatic secretion,nutritional support,fluid resuscitation,maintains the stability of the internal environment in the organism,gastrointestinal decompression if necessary,analgesia and sedation if necessary,anti-infection treatment if necessary.The treatment group was treated with quadruple therapy of traditional Chinese medicine syndrome differentiation on the basis of the control group,it includeds traditional Chinese medicine lavage,traditional Chinese medicine enema,external application of Chinese Medicine,Chinese medicine intravenous drip.Traditional Chinese medicine lavage:Each Chinese medicine decoction 400 ml,through the stomach tube poured into 100 mL each 4 hours,and then clamped the gastric tube,opened it after 1 hour.Traditional Chinese medicine enema: Each Chinese medicine decoction 400 ml,was used for retention enema every 200 ml,once every 6h.External application of Chinese medicine: Chinese medicine was grinded into fine powderfine,mixed with honey herbal compress the upper abdomen and back coverhoney,lotus leaf covered with a bandage,once a day.Chinese medicine intravenous drip: Intravenous drip of breviscapine 100 mg,once a day and the course of treatment was 14 days.Observation items: 1.The time of abdominal pain and abdominal distensionthe disappeared,the time of bowel sound returned to normal;2.The time of laboratory indexes,lipase,C reactive protein,white blood cells returned to normal;3.The MCTSI score before and after treatment;4.The content change of serum TNF-?,IL-6,IL-10 before the treatment,and thirth,severth or fourteenth after treatment.Results: 1.The comparison of the general data of patients: including age,gender,etiology,severity of illness and Traditional Chinese medicine syndrome type comparison,by statistical analysis,P>0.05,there was no significant difference;2.The comparison of the two group in the time of abdominal distension,abdominal pain disappeared and the time of bowel sounds returned to normal,the treatment group was shorter than the control group,there was significantly difference(P<0.05);3.Compared with the time of laboratory indexes,lipase,white blood cell,C-reactive protein returned to normal in the two groups,the treatment group was shorter than those in the control group,there was significantly difference(P<0.05);4.Compared with MCTSI scores of the two groups before treatment,there was no significant difference(P>0.05);MCTSI scores of the two groups after treatment were lower than before treatment,and the treatment group after treatment MCTSI score was significantly lower than that of the control group,the difference was statistically significant(P< 0.05);5.Compared with the concentration levels of serum TNF-?,IL-6,IL-10 in the two groups,the difference was not statistically significant(P>0.05);the concentration levels of serum TNF-?,IL-6 after treatment were lower than those before treatment,IL-10 levels were higher than that before treatment.On the third and seventh days after treatment,the concentration levels of TNF-? and IL-6 in the treatment group were lower than those in the control group,the concentration level of IL-10 was higher than the control group,the difference was statistically significant(P< 0.05).On the fourteenth day after treatment,the concentration levels of serum TNF-?,IL-6 and IL-10 were similar in the two groups,the difference was not statistically significant(P>0.05).Conclusion: 1.Quadruple therapy of traditional Chinese medicine syndrome differentiation could shorten the patient's abdominal pain and distension disappear time,and normal bowel sounds recovery time.2.Quadruple therapy of traditional Chinese medicine syndrome differentiation could shorten the patient's recovery time of AMS,LPS and CRP,and reduce the MCTSI score.3.Quadruple therapy of traditional Chinese medicine syndrome differentiation could reduce the MCTSI score of the patients statistically.4.Quadruple therapy of traditional Chinese medicine syndrome differentiation could early reduce the levels of serum IL-6,TNF-? and increase the level of IL-10.
