Hepatitis B,caused by hepatitis B virus(HBV),is a serious liver disease worldwide.The clinical sysdromes of hepatitis B include inflammatory lesions and multiple organ damage.It is estimated that 1/3 population,around 2 billion people were infected by HBV.Amongst them,about 350 million people were chronically infected by the virus,and more than 750 thousand people died of hepatitis B and its related diseases every year.HBsAg,HBs Ab,HBe Ag,HBe Ab,HBc Ab are the main serological markers for HBV infection.However,until now,seldom studies were conducted to investigate the changing characteristics of these five markers in Chinese adults.Therefore,in order to provide more reliable evidences for the further prevention and control of HBV infections and transmissions,we thoroughly analyzed the changing trends of five HBV serological markers in 3194 individuals undergoing annual physical examinations from 2009 to 2013 in Wuxi,Jiangsu,China.Objective: To analyze the changing characteristics of HBsAg,HBs Ab,HBe Ag,HBe Ab,HBc Ab in 3194 Chinese adults undergoing annual physical examinations from 2009 to 2013.The reults should provide more help for the further prevention and control of HBV infections and transmissionsMethods:From January 2009 to December 2009,3194 Chinese adults undergoing annual physical examinations were participated in the present study after provided the signed informed consent.The exclusion criteria as the following: severe infection,cancer,hyperthyroidism,and other malignant diseases.Additionally,the individulas without completed information and did not participate in the following-up survey were also excluded.Age,sex,smoking,drinking and other demographic characteristics of participants were collected at baseline.HBsAg,HBs Ab,HBe Ag,HBe Ab,HBc Ab were measured using the commercial kits odered from Ke Hua biological company,respectively.HBsAg,HBs Ab,and HBe Ag were detected by double antibody sandwich assay,and HBc Ab and HBe Ab using competitive inhibition assay,respectively.Addionally,Liver function,renal function,blood glucose,blood lipid and other biochemical items were measured by the automatic biochemical analyzer AU5400,The blood routine was measured by the XE5000 automatic blood cell analyzer of Sysmex(Japan).SAS 9.2 software was used for statistical analysis of data.Data were expressed as means ± SD or frequencies.The unpaired t-test and the ?2-text with Yates' s correction for continuity(for categorical variables)were used to compare the characteristics of participants.All analysis using two-sided test,significant level testing for the definition is P< 0.05,Result:1)Totally,3194 individuals were included in the present study with 19-92 years of old.At the baseline,the positive rate of HBsAg,HBs Ab,HBc Ab,HBe Ag and HBe Ab was 6.45%,46.43%,21.35%,0.22% and 14.21%,respectively.The prevalences of the HBsAg,HBc Ab and HBe Ab were significantly higher in males.However,no significant differences of HBs Ab and HBe Ag positive rates between males and females.2)Between 2009 and 2013,two persons developed seropositive for HBsAg.Meanwhile,HBsAg were remissed in 19 cases with HBsAg at baseline.79 persons developed HBs Ab(5.2%)and 107 individuals losted HBs Ab(7.2%).In addition,219 HBc Ab positive cases showed negative transformation(45.6%),while 128 cases showed postive transformation(5.1%).As the results shown,one HBe Ag positive case lost HBe Ag and two HBe Ag negative cases showed positive transformation.45 HBe Ab positive cases losted antibody(16.5%)and 214 cases got HBe Ab during follow-up(7.9%),respectively.3)HBsAg positive-to-negative transformation was negatively correlated with the level of high density lipoprotein(OR=4.45,95%CI: 1.41-14.05).Additionally,HBs Ab positive-to-negetive transformation was associated with a small amount of alcohol(OR=0.32,95%CI: 0.18-0.55).As the results shown,HBc Ab positive-to-negative transformation was negatively correlated with age.The participants with less than 52 year old had a higher HBc Ab seroconversion rate than those of the persons with more than 52 years(OR=0.58,95%CI: 0.39-0.85).In addition,HBe Ab positive-to-negative transformation was negatively correlated with the level of high density lipoprotein(OR=2.88,95%CI: 1.1-7.57).However,HBe Ab negative positive transfer was associated with a small amount of alcohol drinking(OR=1.94,95%CI: 1.28-2.93).Conclusions 1)The prevalence of HBV infection is still higher in Chinese adults.Compared to the females,males have higher prevalence of HBsAg carrier.In addition,the rate of HBsAg positive-to-negative transformation was lower in chronic hepatitis B patients 2)HBsAg and HBe Ab positive-to-negative transformation were negatively associated with HDL levels.Additionally,a small amount of alcohol might delay the decrease of the HBs Ab and promote the production of HBe Ab.However,age is a risk factor for HBc Ab positive-to-negative transformation.
