Purpose:Through the analysis of conditions in Prenatal examination and delivery period,to know the health conditions of pregnant women in Jingjiang,to explore the affecting factors of maternal health,risk factors of pregnancy complication and its morbidity,and to provide Scientific basis for the early intervention.Accordingly,we can find out suitable measures to assure maternal and infant health.Methods:1.Studying Object:This study is based on the integrative management platform system of maternal and children health care in Jingjiang.It enrolls women who were living in Jingjiang and registered perinatal health care card to participate in the health care from January to December in 2014.2.Survey Content:A retrospective cohort study was used to collect factors of maternal health and participants' demographic characteristics including age,education,occupation,hukou,times of prenatal examination,pregnancy history and mode of delivery.Risk factors of pregnancy complications including history of abortion,uterine scar,accessories operation,maternal abnormal height and weight,history of perinatal mortality and complicated disease of pregnancy(hyperthyroidism,diabetes,primary hypertension,heart disease,et al)were also extracted.Test results of HIV,HBV and srphilis,pregnancy outcomes,and suitable health care measures were investigated.3.Statistical analysis:The rate and proportion were used to describe factors of maternal health conditions and distribution of risk factors of pregnancy complications respectively.Chi-square test or Fish's exact probability method was used to compare the differences of affecting factors among different age,education,occupation,hukou,history of pregnancy groups.In pregnancy complications,incidence rates of maternal and infant factors,risk factors such as anemia in pregnancy,pregnancy-induced hypertension,gestational diabetes mellitus(GDM),and viral hepatitis as well as choices of the mode of delivery were included.Results:From January to December in 2014,3,180 casea in Jingjiang were collected.2844(89.4%)of them were confined in hospital.and 175(5.5%)were in other towns,124(3.9%)had an abortion,32(1.01%)had stillbirth before delivery and 5(0.16%)had still birth after delivery.1.Maternal Health Condition and Distribution of Affecting Factors.Among the 2,844 pregnant women having delivery in hospital,the ages of 42(1.48%)case were below 20,900(3.16%)were from 20 to 25,1,152(40.51%)were from 26 to 30 and 750(26.37%)were over 30.The rate of pregnant women with Junior high school and below was 23.35%,with high school education was 32.7% and with college degree and above education was 43.95%.1,640(57.67%)pregnant women have fixed occupation and the others don't have.920(32.35%)had rural hukou and 1,924(43.37%)had urban hukou.1,100 women had a history of previous pregnancy and 1,744 didn't have.Pregnant women with less than 8 times of prenatal examination accounted for 22.86%,and the rest did prenatal examination over 8 times.2.The differences of Affecting Factors in pregnancy ComplicationsThere were 750 cases with ages of over 30,480 of which had pregnancy complications and its incidence rate was 64%,which is obviously higher than that of other pregnant woman of other age groups.The pregnancy complications rate of pregnant women with Junior high school and below was 67.77%,with high school education was 47.1% and with college degree and above education was 44.16%.520(31.71%)pregnant women had fixed occupation and the others didn't have.560(60.87%)had rural hukou and 880(45.74%)had urban hukou.660(60%)woman had a history of previous pregnancy,and 780(44.72%)didn't have.Pregnant women with less than 8 times of prenatal examination.accounted for 62.81%),and the rest did prenatal examination over 8 times.The incidence rate of well-educated pregnancy women was lower than that of ill-educated pregnancy women.The maternal with fixed occupation had lower morbidity of pregnancy complications than those without fixed occupation.Those with rural hukou had higher incidence of pregnancy complications than those with urban hukou.Women with a history of previous pregnancy,experiencing<8 of prenatal examination during the pregnancy had higher morbidity of complications than those without history of pregnancy,or experiencing ?8 times prenatal examination.3.The Differences of Affecting Factors of anemia in pregnancy,pregnancy-induced hypertension,gestational diabetes mellitus in pregnancy complications.Regard to the 550 pregnant women having pregnancy complications at the first three places,as for anemia in pregnancy,the maternal with rural hukou occupied 86.2%(250),and with urban hukou occupied 51.9%(135).As for pregnancy-induced hypertension,the maternal with rural hukou and urban hukou respectively made up 7.6%(22)and 27.7%(72).As for GDM,the rural constituted 6.2%(18)and the urban constituted 20.4%(53).The incidence rate of anemia in pregnancy of rural women was higher than that of urban women(86.2% vs 51.9%,p<0.001).The incidence rate of pregnancy-induced hypertension of rural women was lower than that of urban women(27.7% vs 7.6%,p<0.001).The incidence rate of GDM of rural women was lower than that of urban women(20.4% vs 6.2%,p<0.001).Conclusion:1.The differences in affecting factors of maternal health condition,and pregnancy complications are at a normal level.2.The health condition of the maternal is linked to factors such as age,education,occupation,hukou,history of pregnancy,times of prenatal examination,etc.In order to decrease the incidence rate of high-risk pregnancy,the education of the maternal health care should be strengthened,and people's awareness should be roused.3.The maternal and infant adverse outcomes were increased by pregnancy complications,leading to a series of fetal diseases.The management of maternal health should be strengthened to prevent and reduce the occurrence of pregnancy complications,and to reduce maternal and neonatal complications and death.
