Objective:To explore the recent clinical efficacy of TVT-O in the treatment of female stress urinary incontinence and the changes in urodynamic parameters before and after the operation.Methods:The clinical data of 88 cases of female stress urinary incontinence(SUI)treated with TVT-O in Department of Urology of the first affiliated hospital of Nanchang university from June 2015 to June 2017 were analyzed retrospectively in this article.All patients were required to fill in urinary incontinence quality of life questionnaire(I-QOL)and international consultation committee on incontinence questionnaire short form(ICI-Q-SF)and make a urodynamic examination before the operation and at the time of follow-up.The general data of patients,the operation time,intraoperative complications,postoperative hospital stay and postoperative ureteral indwelling time were recorded and followed up for 6 weeks after operation to analyze and evaluate the postoperative subjective efficacy,complications,urine improvement of incontinence quality of life and change of urodynamic parameters.Results:The operation of 88 patients was completed successfully.The operation time was 15-35 min,with an average of(24.7±6.5)min,the bleeding volume was 5-30 ml,the average was(20.4±8.4)ml,and the postoperative hospitalization was 1-4 days,with an average of(2.3±0.9)days.Postoperative catheter was kept for 1-8 days,with an average of(1.7±1.1)days.No serious complications,such as urethra,bladder,intestinal canal injury and massive hemorrhage,were found during the operation.82 cases(93.2%)were completely cured,6 cases(6.8%)were improved and 0 cases were ineffective.A total of 5 patients(5.7%)had postoperative dysuria after catheter removal,and 2 patients(2.3%)had frequent urination and urinary urgency after removal of catheter.3 patients(3.4%)had mild pain at the thigh root,and all of the above patients,symptoms improved after the related treatment.After 6 weeks postoperative follow-up,the I-QOL score showed that the quality of life of patients increased significantly(P <0.05),the ICI-Q-SF score indicated that the symptoms of urinary incontinence were significantly improved(P <0.05),and the urodynamic examination showed that in addition to lower than the preoperative difference in maximal urinary flow rate and average urinary flow rate was statistically significant(P<0.05),the postvoid residual volume,maximum detrusor contraction pressure,initial capacity,strong capacity of urination,the maximum urethral pressure,maximum urethral closure pressure and functional urethral length were compared with the preoperative,there was no significant difference(P>0.05).Conclusions:TVT-O is safe and reliable in the treatment of female SUI,and the curative effect is significant.
