| Objective:To evaluate the utilization of the high-resolution magnetic resonance(HR-MR)in intracranial artery dissection aneurysms.Methods:We selected the patients whom was diagnosed intracranial artery dissection aneurysms during December 2015 and December 2017 in the database of our department.It was divided into conservative observation group and surgical treatment group according to whether the endovascular intervention was performed.Those who were followed up with high-resolution magnetic resonance(HR-MR)and DSA(interval time<7days)were selected to study.We use the TOF-MRA and the high resolution magnetic resonance vascular wall imaging(HR-VWI)to observe the dissection aneurysm imaging of surgical treatment group on T1-and T2-weighted sequences,dissection aneurysm wall enhancement imaging and the arterial stenosis.And all the results were compared with the results of DSA.Results:There were included 29 patients,24 males and 5 females.The average age was 51.77 + 13.08 years.Among them,there were 17 cases of vertebral artery dissection aneurysms,9 cases of basal artery dissection aneurysms,2 cases of internal carotid artery dissection aneurysm,and 1 case of middle cerebral artery dissection aneurysms.There were 16 cases in the operation group and 13 in the conservative treatment group.14 patients were followed up(9 in the operation group,5 in the conservative treatment group),and the follow-up rate was 48.27%.In the conservative treatment group,4 patients were followed up,1 patient underwent surgery after the first follow up.10 patients of operation group were follow up(9 patients vertebral artery dissection aneurysm,1 patient basilar artery dissection aneurysm).They totally had 13 imaging follow-up with DSA and HR-MRI,and the mean follow-up time was 6.6±5.6 months(range2-22 months).There were 9 parent arteries with stenosis and 4 parent arteries without stenosis in TOF-MRA;4 with stenosis and 9 without stenosis in HR-VWI;3 with stenosis and 10 without stenosis in DSA,respectively.There were 3 patients with recurrence in HR-VWI,TOF-MRA and DS A.Conclusions:1.High resolution magnetic resonance(HR-MRI)can provide detailed information of intracranial arterial dissecting aneurysm and vascular wall,which can be used as a auxiliary tool to diagnosis the intracranial arterial dissecting aneurysm.2.The HR-MR can be used as an effective follow-up method for the endovascular treated vertebrobasilar dissection aneurysms.The findings include the aneurysm demonstrate hyperintense on T1-weighted imaging,slightly hyperintense on T2-weighted imaging;strong enhancing in the dissection aneurysm wall or aneurysm cavity on contrast-enhanced T1-weighted image,the blurred boundary of the artery wall and double-lumen in HR-VWI imaging can be used to determine the recurrence of dissection aneurysms. |
