Objective:Through the study of sodium hydrosulfide?NaHS?on the impact on blood coagulation factors in rabbit model of urosepsis,to further explore the protective mechanism of H2S from the source of urinary sepsis,and provide experimental basis for clinical application of H2S in urine derived sepsis.Materials and methods:30 healthy male New Zealand rabbits were selected,weighing 2.0-2.2kg,were randomly divided into 3 groups,10rabbits in each group:group Sham?sham operation?in rabbits after caesarean section,only the left middle ureter free.Sepsis?urosepsis?group:free and ligation of rabbits left middle Ureter Ligation to.Proximal ureteral lumen infusion of Escherichia coli bacteria?0.5ml/Kg?,.HS-Se?proximal ligation injection of NaHS in urosepsis model?group:operation with the Sepsis group,after ear vein according to8.4 mol/Dose concentration of kg solution of NaHS 50mmol/L.All the groups at preoperative,postoperative 24h,48 h and 72h by ear vein blood platelet count,plasma antithrombin?activity?AT?:A?,plasminogen activity?PLG:A?,coagulation factor V?activity?FV?:C?,von Willebrand factor?v WF?activity,the expression level of PAR-1,pathology and renal function.After the data were obtained using SPSS19.0 software to process and analyze the data.Results:1.The postoperative state of the sham group can be seen without bleeding spots in the skin and mucosa.Sepsis group mucosa bleeding after operation of skin,hair color was not smooth,shortness of breath,drinking less,refused to eat,unresponsive to external stimuli;72h after 7 cases were chills,2 cases were bleeding spots in the skin and mucosa.HS-Se group were visible small bleeding after the spirit of poor hair color less smooth,slightly out of breath without promoting,chills and skin mucosa bleeding,eating and drinking is acceptable to external stimuli responsive.2.platelet:sepsis group after 24 h,48 h and 72 h compared with sham group,was significantly increased?P<0.05?after operation;and significantly higher than group HS-Se in48h,72h?P<0.05?.HS-Se group after 24h,48h and 72 h significantly higher than that in sham group?P<0.05?.3.antithrombin activity?AT III:A?:sepsis group after 24 h,48 h and 72 h were significantly lower than those in sham group?P<0.05?;postoperative 48h was lower than that of group HS-Se?P<0.05?.HS-Se group after 48h and 72h when compared with sham group,the difference was statistically significant?P<0.05?.4.plasminogen activity?PLG:A?:sham group,HS-Se group after 24 h,48 h and 72 h respectively compared with preoperative patients,there was no statistically significant difference?P>0.05?.Sepsis group were significantly lower than those in other groups after 72h?P<0.05?.5.von Willebrand factor?vWF?:sepsis group were compared between groups were significantly higher than those in group sham and group HS-Se?P<0.05?.6.protease activated receptor?PAR-1?:There was no significant difference between intra group comparison?P>0.05?.Compared with the sham group,the sepsis group increased significantly?P<0.05?;compared with group HS-Se,group sepsis were significantly higher than those of group HS-Se and group in 48h and 72h?P<0.05?.Sepsis group,postoperative 72h and the other 2 groups were significantly increased.7.groups before operation?BUN,Cr?showed no significant difference?P>0.05?.Sepsis group after the operation of 72h and the other 2 groups were significantly higher?P<0.05?.8.After renal tissue HE staining,there were no obvious abnormalities in renal morphology in sham group.The sepsis group showed extensive renal tubule,large suppurative change,a large number of neutrophil infiltration and tube type.HS-Se group showed renal tubular epithelial edema,degeneration,part of the region without obvious purulent changes,visible small part of inflammatory cell infiltration.Conclusion:1.urosepsis can lead to a series of changes of blood coagulation function:increased platelet count in short term,increased expression of vWF and PAR-1,decreased AT III:A and PLG:A.2.H2S can inhibit the rising trend of platelet in sepsis,inhibition of vWF expression,PAR-1 activity,while AT-III:A and PLG:A can reduce the hydrogen sulfide of urosepsis consumption in the dynamic balance of the blood coagulation system stability3.H2S has a certain protective effect on renal function in sepsis,and reduce renal microvascular thrombosis on the need to be further clarified.
