Background:Oxidative stress and inflammation related endothelial dysfunction is involved in the pathogenesis and progression of coronary artery disease(CAD).Measurement of 8-isoprostane and high-sensitive C-reactive protein(hs-CRP)has emerged as reliable approach to assess oxidative stress status and inflammation status in vivo.The aim of the study was to evaluate the association between urinary8-isoprostane,plasma hs-CRP concentration and Syntaxscore in patients with CAD.Methods: Urinary 8-iso-PGF and plasma hs-CRP were measured in112 patients of angiographically confirmed CAD.Syntaxscore was used to evaluate the severity of coronary lesions.The associations between urinary 8-isoprostane,plasma hs-CRP concentration and Syntaxscore were evaluated by Spearman correlation analysis.Our study included 27 subjects with coronary artery Syntaxscore??33(group 1),27 patients with 22<Syntaxscore?<33(group 2),and 58 subjects with Syntaxscore??22(group 3).Results: Urinary levels of 8-iso-PGF were group 1: median 244(149,300)pg/mmol?Cr;group 2:77(56,107)pg/mmol?Cr;group 2:73(51,114)pg/mmol ? Cr(P < 0.05),while the plasma levels of hs-CRP were median 1.9 quartile(0.8,5.0)mg/L;0.9(0.7,2.7)mg/L;1.0(0.4,2.3)mg/L in groups 1,2 and 3respectively(P>0.05).Spearman correlation analysis indicated that 8-iso-PGF concentrations were positively associated with the severity of CAD as evaluated by the Syntaxscore?(r= 0.572,p<0.001),but were not correlated with Syntax?(PCI)(r=0.184,p=0.142)or Syntax ?(CABG)(r=-0.139.136,p=0.271).Plasma hs-CRP concentrations were not correlated with Syntax?(r=0.133,p=0.244)?Syntax?(PCI)(r=0.181,p=0.116)or Syntax?(CABG)(r=-0.052.136,p=0.652).Conclusion: Urinary 8-iso-PGF levels are positively correlated with Syntaxscore,which may be reflective of the association between the extent of oxidative stress and the severity of coronary lesions.
