Object:The paper investigate the prognostic factors on advanced primary liver cancer patients with hepatitis B surface antigen positive in order to ulteriorly explore the influence of HBV viral load and antiviral treatment on the prognosis of advanced liver cancer and provide the basis for clinical treatment and evaluating prognosis.Methods:A total of 95 cases with complete data were selected from January 2010 to December 2013 in our hospital.Application of life table method to calculate survival rate.Statistical analysis was carried out on the gender,age,cirrhosis of the liver,AFP,liver function of Child-pugh classify,the number of tumor,tumor size,tumor shape,peritoneal lymph node metastasis,the distant organ metastasis,BCLC,portal vein tumor thrombus,therapeutic method,number of TACE treatment,HBV viral load and against hepatitis B virus treatment.Single-factor analysis and multivariate analysis was performed on prognostic factors using Log-rank test and Cox regression analysis to select independent prognosis factors of hepatitis B-associated advanced liver cancer.Results:1.The are totally 95 patients in the study in which there are 72 male patients and 23 female patients and their ages range from 37 to 82.Themedian survival time was 10 months and the 1-,2-,3-and 5-year overall survival rates was 39.0%,17.9%,8.40%and 1.05%,respectively.2.Single factor analysis showed Alpha fetoprotein,liver function of Child-Pugh,tumor morphology,the distant organ metastasis,BCLC staging,portal vein tumor,therapeutic method,frequency of TACE treatment and antiviral therapy were the relative factors.3.Prognostic factors in multivariate analysis were AFP,the distant organ metastasis,BCLC staging,portal vein tumor thrombus,therapeutic method,frequency of TACE treatment and antiviral therapy were the relative factors.Conclusion:1.High concentration of AFP,the distant organ metastasis,Barcelona clinic liver cancer staging later and portal vein tumor thrombus are the independent risk prognostic factors.2.Antiviral therapy can prolong the survival of advanced PLC patients with HBV-DNA positive.3.TACE is a safe and effective treatment and appropriate retreatment of TACE can prolong the overall survival time.
