Application Of Tracheotomy Tube As Trocar In The Electronic Bronchoscope Instead Of Thoracoscope

Objective:1.To investigate the feasibility of the disposable tracheotomy tube as Trocar in the electronic bronchoscope instead of thoracoscope.2.In the Department of thoracic mirror,compared with the conventional thoracic wall casing,use disposable tracheotomy catheter as the chest wall casing,whether can shorten the operation time,whether can improve the diagnostic rate,whether to reduce costs.There was no significant difference in the incidence of complications,preoperative and intraoperative arterial blood gas analysis.Methods:A study was performed between January 2015 and October 2015 in Shaoyang central hospital Respiratory Medicine.86 patients,59 male(68.6%)and 27 female(34.1%)and mean age(49±20)years(15~83)years,with effusion of unknown origin underwent medical thoracoscopy.86 patients were randomly divided into two groups,One group uses a disposable tracheotomy tube as trocar,Another group received conventional medical thoracoscopy chest tube,Two groups of patients in our hospital or outside the hospital were pleural puncture drainage,pleural effusion biochemical,routine,cytology,tumor markers,etc,were not clear diagnosis,families and patients with a strong demand for a clear diagnosis and refused to carry out a diagnosis of anti tuberculosis treatment.To compare the positive rate of pathological diagnosis of both,operation time,inspection fees,blood gas analysis and other adverse reactions.Results:1.43 cases of patients with tracheotomy tube as chest wall in cannula the use of electronic bronchoscope,pH,PaO2,PaCO2,SaO2 have no significant difference(P >0.05).2.Two groups of patients with pathological positive rate,operation time,there were significant differences.tracheotomy tube group 43 cases diagnosed in 40cases(93%),including lung pleural metastasis in 8 cases,31 cases of Tuberculosis pleurisy,emphysema 1 case,four cases of unknown diagnosis(7%).Conventional chest tube group 43 cases diagnosed in 33cases(76.7%).including lung pleural metastasis in 4 cases,29 cases of Tuberculosis pleurisy,10 cases of unknown diagnosis(18.6%).two groups,the difference was statistically significant(P<0.05).Disposable catheter tracheotomy group operative time was(23.86±2.45)minutes,Conventional chest tube group operation time(29.88±3.67)minutes,two groups,the difference was statistically significant(P<0.05).The price of endotracheal tube was significantly lower than that of conventional chest wall.The complication rate was no significant difference.Conclusion:1.Transthoracic puncture,pleural effusion routine,biochemistry,bacteriology,cytology,tumor marker examination was not diagnosed patients,can improve the positive rate of diagnosis by medical thoracoscopy,has clinical value,which is worthy of wide application.2.The use of disposable tracheotomy tube as chest wall cannula can shorten the operation time,improve the diagnostic positive rate and reduce the cost.
