Objective:To explore the outcome of mild or moderate functional mitral regurgitation after aortic valve replacement and risk factors,clear postoperative recovery,the results guide the clinical work.Methods:We retrospectively analyzed 132 patients with aortic valve replacement who had mild or moderate FMR before surgery from 2005 to 2012,and identified the clinical characteristics of these patients.All patients were followed up for preoperative,intraoperative and perioperative clinical data.The main observation indicators for the patient echocardiographic results,while access to ECG,chest radiographs,assessment of patients with heart function changes and quality of life.Result:Perioperative mortality was 3.8%.During the follow-up period,10 patients died and 97 patients were followed up.81%of patients with FMR improved after aortic valve replacement.Compared with FMR group(group B),there were significant differences in the left ventricular diameter,pulmonary artery pressure,atrial fibrillation and BNP level between the FMR group and the FMR group(P<0.05).The difference between the two groups was statistically significant Disease,left ventricular EF,left ventricular end diastolic diameter,CPMA,preoperative mitral regurgitation and preoperative aortic valve disease type.Multivariate regression analysis showed that left atrial diameter,pulmonary artery pressure,preoperative combined with atrial fibrillation after FMR can not improve the independent risk factors.The ROC curve to cut the value found that the left atrial diameter of 49.5mm,pulmonary artery pressure reached 51.5mmHg,preoperative combined with atrial fibrillation can predict the long-term prognosis of FMR.The quality of life and survival of the FMR improvement group were significantly higher than those of the FMR group during the 5-year follow-up period.Conclusions:Most mild to moderate or moderate FMR in the aortic valve replacement over the same period can be conservative observation,and left atrial diastolic diameter,pulmonary artery pressure,atrial fibrillation is FMR postoperative risk factors.Postoperative residual FMR significantly affected patients with postoperative survival and quality of life.
