Good nutritional support after esophageal cancer surgery has become an important guarantee for the success of esophageal cancer surgery and improve the quality of life.The early postoperative enteral nutrition can not only meet the needs of postoperative stress,but also can not cause the further increasing of malnutrition due to absorption dysfunction.However,a number of studies have shown that patients with early enteral nutrition may have a variety of clinical manifestations of feeding intolerance,which seriously affect the quality of life and prognosis of patients with esophageal cancer.The concept of rapid rehabilitation surgery which was first made on Copenhagen Denmark Kehlet,is the new concept of the latest research of surgery,anesthesia and nursing integration,and is to adopt the optimization measures to reduce patient trauma,promote early rehabilitation of patients.According to the principle of rapid rehabilitation surgery,comprehensive intervention program was developed by integrating series of effective measures to improve the symptoms of early enteral feeding intolerance in patients.This study collected from January 2015 to August 2015 at a three level of first-class hospital treatment of 93 cases of esophageal cancer patients,according to admission time sequence,from January 2015 to April 2015 using the traditional control scheme,from May 2015 to August 2015 the application of comprehensive intervention program was to evaluate the clinical effects of enteral nutrition on raising control tolerance and comprehensive intervention program.ObjectivesAccording to the principle of rapid rehabilitation surgery,the therapeutic effect of the comprehensive intervention program in the intestinal nutritional tolerance of esophageal cancer patients was confirmed.MethodsWith the concept of rapid rehabilitation surgery as the principle to develop a series of perioperative comprehensive intervention program,the preoperative plan,including preoperative health education,postoperative functional exercise,not restricted diet and oral carbohydrate;including surgery,intraoperative warming scheme:anesthetic optimization;postoperative early catheter removal,solutions include:early,early enteral nutrition.In January 2015-2015 year in August in a tertiary hospital of 93 patients with radical resection of esophageal carcinoma,all patients prescribed for early enteral nutrition,were divided randomly into the intervention group and the conventional control group,intervention group were treated with comprehensive intervention,the control group were given routine nursing care,compared two groups enteral nutrition in patients with intolerance incidence,for the first time and the time of bowel sounds,smell the first exhaust time,first defecation time and postoperative complications and other clinical indicators.ResultsThe intervention group and conventional control group,intervention group in 47 cases,36 cases(76.60%)the tolerance of early enteral nutrition support,the 46 patients in the control group with 24 cases(52.17%)tolerance of early enteral nutrition support,there was statistically significant difference between the two groups(P<0.05)the intervention group patients;the first exhaust time,smell and time of bowel sounds,the first defecation time,hospitalization time were better than the conventional control group,there was statistically significant difference(P<0.01);and the comprehensive intervention group during hospitalization,incision infection,pulmonary infection,intestinal obstruction incidence were lower than those of the control group,there was significant significant differences between the two groups(P<0.05).ConclusionsWith the concept of rapid rehabilitation surgery,the comprehensive intervention principle plan is safe and effective.Compared with the traditional treatment,the comprehensive intervention program could effectively reduce the enteral nutrition intolerance incidence,promote the body recovery,shorten hospital stay,reduce the incidence of postoperative complications,and improve patients' quality of life after surgery.
