Objective Congenital hypothyroidism?CH?of newborn is one of the common endocrine diseases in children,if not being treated promptly,they will cause serious nervous system and physical development injury.Analysis of newborn congenital hypothyroidism screening in HengYang from 2013 to 2015,to investigate the prevalence rate and risk factors of CH for provide guidance of the treatment of congenital hypothyroidism in HengYang area and provide data for further study of HengYang area.Method Using the method of retrospective investigation,207481 cases of newborn congenital hypothyroidism screening in HengYang?seven counties and five districts?from 2013 to 2015 related statements descriptive analysis and case information and statistical inference.Investigation and detection methods:adopting the time-resolved fluorescence immunoassay in the detection of neonatal serum thyroid stimulation hormone?TSH?value,TSH>10.0uIu/ml means the screened results are positive and needs to be recall for review or further diagnosed.Venous blood of children with abnormal values by chemiluminescence's method to detect serum TSH?FT3,FT4,T3,T4 concentrations,and take it with ultrasound examination of thyroid,bone ages scan and thyroid radioisotope scan,etc.The child whose serum TSH was higher than normal,FT4 below the normal,was diagnosed of Congenital hypothyroidism.And used 1:1 case-control study and multiple factors were investigated in 60 pairs of parent?mainly for the mother?,using?2test for the mother's age,family history,births,family income factors that were studied by means of single factor analysis;The single factor analysis was statistically significant factors for multi-factor Logistic stepwise regression analysis.Inspection level for a=0.05.Results 1.At HengYang region in 2013 to 2015,207481 cases had done the neonatal screening,60 cases were confirmed CH,the positive rate is28.92?1/10 ten thousand?.To compare the birthplace,HengNan county's incidence of CH is significantly higher than the other counties,the difference for both region's incidence of CH with statistical significant.The incidence of CH has seasonal correlation,summer and autumn fall below the winter and spring,but no statistics difference.2.Results for single factor analysis:With rural household registration,Older mothers,have a family history of autoimmune thyroid disease,and premature,postdates gestations,low birthweight,and twins or multiple births have the incidence for CH.with statistically significant?P<0.05?.3.Results for multi-factor Logistic stepwise regression analysis:logistic?P?=-1.796-1.388X1+1.273X2+2.077X3,With rural household registration?X1?,Older mothers?X2?,have a family history of autoimmune thyroid disease?X3?,are the high risk factor for CH of newborn.Conclusion 1.At HengYang region in 2013 to 2015,the positive rate of CH is 28.92?1/10 ten thousand?.This result is close to the national average.HengNan county's incidence of CH is the highest than the other counties.2.The incidence of the CH have related with the residential place,child-bearing ages,whether have a family history of autoimmune thyroid disease,Neonatal birth weight,gestational age,gestational age and births.3.Results for multi-factor Logistic stepwise regression analysis:logistic?P?=-1.796-1.388X1+1.273X2+2.077X3,With rural household registration?X1?,Older mothers?X2?,have a family history of autoimmune thyroid disease?X3?,are the high risk factor for CH of newborn.
