ObjectiveIn this paper, our main objective is to develop a scientific, reliable and effective Transference Criteria Assessment Form, which becomes a valuable quantification tool for ICU doctors and nurses in assessing the ICU patient whether or not suitable for safe transference.Methods1 The formation of the initial scale: the framework of Transference Criteria Assessment Form for ICU Patients was initially defined by the research group on the basis of consulting extensive literature at home and abroad and analyzing retrospectively many clinical cases with APACHE-? score. Then, thirty domestic experts, including 28 ICU clinical medical experts and 2 ICU clinical nursing experts, were selected to join in the consultation. The index system of Transference Criteria Assessment Form for ICU Patients and a primary scale with 2 dimensions and 9 items were established after two rounds of expert consultation and appropriate revisions. After entries for the assignment, the Initial Transference Criteria Assessment Form for ICU Patients was established.2 The formation of the formal scale: 493 ICU patients from a tertiary hospital were picked out by cluster sampling and tested with Transference Criteria Assessment Form for ICU Patients(test edition)and APACHE-? score. Then we carry on the statistical analysis to the obtained dada by using the SPSS 22.0.Based on data analysis results, the items on the competence instrument were retained or deleted after item analysis.Content validity, structure validity, internal consistency reliability and split-half reliability were used to examine reliability of the scale.Consequently, the final and formal Transference Criteria Assessment Form for ICU Patients were formed.3 Applying effects: apply the formal form to rescore 493 ICU patients, determine the threshold value for safe transference and validate effectiveness clinically of Transference Criteria Assessment Form for ICU Patients.Results1 The formed Transference Criteria Assessment Form for ICU Patients includes 2 dimensions and 9 items:(1)basic information consisting of 4 items;(2)physiological indexes consisting of 5 items.2 The validity and coefficient of the Transference Criteria Assessment Form for ICU Patients. The content validity index(CVI)for the overall scale and each dimension ranged from 0.833 to 1.00;An exploratory factor analysis showed that factor loading are both greater than 0.35.The correlation coefficients between the two dimensions and the total scale and the external criterion were 0.421~0.841(P<0.01).The scale showed good construct validity. The correlation coefficient between the total scale and 2 dimensions and the external criterion was 0.307~0.412(P<0.01).Two indexes showed a statistically significant correlation. The scale showed good criterion-related validity. The correlations among the two dimensions were statistically significant(P<0.01).The structure validity of the scale was also good. For overall scale, the Cronbath's ? coefficient was 0.752.The Cronbath's ? of the scale were 0.717~0.775 and the split coefficient was 0.689.The reliability of the scale was good.3 The application results showed that areas under the ROC curve of Transference Criteria Assessment Form for ICU Patients and APACHE-? score were 0.957 and 0.821,respectively,and standard error were 0.012 and 0.032,respectively(P=0.000).Safe transference percentage in Group scoring(?7)and Group scoring(<7)were 82.95% and 17.05%(P=0.000),showed some statistically difference. Therefore, point 7 is determined as the threshold value of Transference Criteria Assessment Form for ICU Patients.ConclusionsThe formed Transference Criteria Assessment Form for ICU Patients has good reliability and validity and therefore it can be used as a valuable quantification tool to assess and guide transference of ICU patients.
