| ObjectiveTo investigate the prevalence of hypertension among residents aged 15 years and older in Hefei city.On the basis of analyzing the effects of the general demographic factors(including age,gender,marital status,employment status,etc.),lifestyle behavior,dietary factors,past history,family history on hypertension in this area,the residents BMI,waist circumference and body fat The relationship between the rate and the risk of hypertension,the association between BMI,waist circumference,body fat percentage and the risk of hypertension were analyzed..To provide the basis for the selection of indicators of obesity in the prevention and treatment of hypertension.MethodsWith a multi-stage stratified random sampling method,a total of 1900 permanent residents aged 15 and older were randomly selected from six neighborhood committees in Hefei City.We used the methods of questionnaire and physical measurement to obtain detailed target information during June 2014 to December 2014.Prevalence of Cardiovascular Disease in China and Key Technology Research Data Collection and Management System was used for inputing data.SPSS17.0 was used for statistical description and statistical analysis.Influencing factors of hypertension were analysed by t test,X~2 test and logistic regression.Results1.The characteristics of the investigated peopleIn this study,1883 valid samples were recovered and the efficiency was 99.1%.Among them,male were 947,and female were 936.The oldest and the youngest were 95 years and 15 years,respectively,and the average age was(45.66±19.26)years.The averages of BMI,waist circumference and body fat rate were 23.68kg/m~2,83.21cm and 26.52%,respectively.The averages of Systolic and diastolic blood pressure were 123.48mmHg and 73.46mmHg,and the ranges were 81~194mmHg and 45~119mmHg,respectively.The cultural level of most respondents was junior high school,accounting for 30.4%.For marital status,the main group was married,accounted for 76.5%.The main employment situation was employment,accounting for 51.0%.The smoking and drinking rate were 27.2%and 14.9%.According to BMI,the proportion of thin,normal,overweight and obese people were 5.0%,49.7%,34.0%and 11.3%,respectively.According to the size of the waist circumference,the central obesity accounted for34.4%.The investigated people,who had received hypertension management services,coronary bypass surgery and coronary angioplasty or stent implantation accounted for13.9%,0.0%and 0.3%,respectively,and who had been diagnosed as myocardial infarction and stroke accounted for 0.4%and 0.5%,respectively,and there were 39.0%,6.4%,11.2%,3.5%and 4.9%of the total group of people with family history of hypertension,hyperlipidemia,diabetes and stroke,respectively.2.The detection of hypertensionIn the survey population,the prevalence of hypertension was 28.7%,and the standardized prevalence was 21.8%.The prevalence of hypertension in male and female were 32.3%and 25.1%,and the standardized prevalence were 26.4%and 17.2%.According to the age composition of the sixth census population,and for male and female.According to different sex groups,the prevalence of hypertension of male was higher than that of female(X~2=11.937,P<0.05).According to different age groups,the prevalence of hypertension in the population increased with age(X~2=504.508,P<0.05),and men and women presented the same trend.3.The association between hypertension and BMI,waist circumference and body fat rate ByX~2 test,BMI,waist circumference,body fat rate were associated with hypertension(P<0.05).According to BMI,waist circumference,body fat rate were defined obesity,the order of the detection rate of hypertension in obese people was as follows:body fat rate>waist circumference>BMI.4.The association between hypertension and other related factorsIn the general demographic factors,the cultural level,marital status and employment status were related to hypertension(P<0.05).As for different cultural levels,illiterate or semi-illiterate people had the highest prevalence of hypertension,accounting for 59%.As for different marital status,widowed residents had the highest prevalence of hypertension,accounting for 59.0%.Among the different employment conditions,the highest prevalence of hypertension was in the retired population,accounting for 58.1%.As for dietary factors,the average daily intake of staple food,fruit,livestock and poultry meat,milk and pickles were associated with the prevalence of hypertension(P<0.05),and the average daily intake of pickles was higher in the hypertensive group than that in the non-hypertensive group(P<0.05),the others intake were lower than those of the non-hypertensive group(P<0.05).As for related factors in life behavior,smoking,drinking,the weekly number of physical activity(including work,leisure and transport related to physical activity),static behavior time and sleep time were related to the prevalence of hypertension(P<0.05).The prevalence of hypertension of smoking residents(35.5%)and drinking residents(43.6%)were higher.The number of physical activity,static behavior time and duration of sleep of the hypertensive group were less than those of the non-hypertensive group.As for past history and family history,residents,who had received hypertension management services,or been diagnosed as myocardial infarction,or stroke,or family history of hypertension,or coronary heart disease,were at high risk for hypertension(P<0.05).5.Correlation analysis of BMI,waist circumference and body fat percentage with hypertensionMultivariate logistic regression analysis showed that waist circumference and fat rate had effects on hypertension(P<0.05)when the model 1 only included BMI,waist circumference and fat rate.The ORs of waist circumference and body fat rate were2.523(1.963~3.241)and 3.439(2.678~4.416).When controlling age and gender effects or controlling age,gender,dietary factors,lifestyle behavior,past history and family history in the models(model2 and model3),the three indicators all had effect on hypertension(P<0.05).In model 2,the ORs of waist circumference,body fat rate,BMI were 2.070(1.568~2.733),1.555(1.169~2.067),and 1.512(1.243~1.839).In Model 3,the ORs of waist circumference,body fat rate,BMI were 1.767(1.258~2.481),1.709(1.206~2.422)and 1.478(1.167~1.872),respectively.Conclusions1.The prevalence of hypertension in Hefei residents was higher and increased with age.The prevalence of hypertension of male was higher than female.2.BMI,waist circumference,body fat rate were all associated with hypertension.Among the three indicators,waist circumference was the most sensitive indicators for detecting hypertension.3.There are many Influencing factors of hypertension.Urge residents to develop good habits and eating habits awareness,quit smoking and drinking,strengthen physical activity,develop good sleep habits,balance the diet,which can effectively prevent or reduce the occurrence of hypertension and its complications and progress.
