Objective:Evaluation of hemodynamic changes of the middle cerebral artery in preterm infants by ultrasonography,objective to investigate the effect of delayed umbilical cord on premature infants with intracranial hemorrhage.Methods:Preterm infants born in Wuxi Maternal and Child Health-Care Hospital from 2015 to 2016 were selected as research subjects,according to the inclusion criteria,80 patients were randomly divided into two groups,33 cases as experimental group,take 60 S delayed umbilical,47 cases as control group,take the 10 S routine umbilical.The incidence of intracranial hemorrhage and the hemodynamics of middle cerebral artery were monitored by ultrasound,Hemodynamic parameters included peak systolic velocity,end diastolic velocity,pulsatility index and resistance index.Results:(1)There was no significant difference in gestational age,premature birth,gestational age and birth weight between the experimental group and the control group(P>0.05),It shows that the two groups are comparable;(2)The incidence of intracranial hemorrhage in the experimental group(30.3%)was lower than that in the control group(53.2%),the difference was statistically significant(?2=4.127,P < 0.05);(3)There was no significant difference between the experimental group and the control group in intracranial hemorrhage(?2=0.700,P>0.05);(4)The experimental group hemodynamics of middle cerebral artery peak systolic velocity of Vs(41.24±5.90cm/s),end diastolic velocity of Vd(10.45±2.84cm/s),pulsatility index PI(1.61±0.24)were higher than the control group systolic peak velocity Vs(39.21±9.46cm/s),end diastolic velocity of Vd(8.95±4.53cm/s),pulsatility index PI(1.57±0.31),the experimental group of middle cerebral artery resistance index RI(0.73±0.06)is lower than that of the control group resistance index RI(0.76±0.08),the difference was statistically significant(P < 0.05).Conclusion:Premature infants with delayed 60 S can effectively improve the early cerebral hemodynamic indexes,so that the middle cerebral artery in the state of high flow and low resistance,improve the blood supply of the brain;Ultrasound can provide effective and reliable hemodynamic parameters for clinical reference,take effective measures to reduce the incidence of intracranial hemorrhage,better improve the prognosis of premature infants.
