Objective: To detect the expression of macrophage migration inhibitory factor(MIF)and transforming growth factor-?(TGF-?)in the serum of IgA nephropathy patients,and to explore the possibility of serum MIF levels combined with TGF-?levels to assess the degree of IgA nephropathy pathological damage and prognosis,provides a theoretical basis for clinical anti-MIF and anti-TGF-? therapy,and seeks for non-invasive testing methods that can accurately judge the degree of pathological lesions and prognosis for patients requiring repeat renal biopsy.Methods: The experimental group selected 60 patients with IgA nephropathy who had been diagnosed as having renal puncture biopsy,fasting cubital venous blood in the morning,and 30 healthy volunteers in the control group had fasting cubital venous blood in the morning.The clinical data of two groups of subjects were collected,such as 24-hour urine protein(TUP),serum creatinine(Scr),glomerular filtration rate(GFR),blood pressure(BP),hematuria,and the like.All patients underwent percutaneous renal biopsy under the guidance of B-ultrasound.Renal tissue specimens were examined by light microscopy,electron microscopy,and immunofluorescence at the Department of Nephrology,The First People's Hospital Of Yunnan Province.On the basis of the different degree of glomerular and tubulointerstitial damage,According to Lee's criteria,it is graded I,II,III,IV,and V.The morning phlebotom of the patient on the day of renal biopsy was taken and the upper serum was collected by centrifugation.The MIF and TGF-?1 concentrations were detected by ELISA.The operation was performed strictly according to the kit instructions.Correlation analysis was used to analyze the correlation between MIF and TGF-?1 and clinical data.All data were analyzed using SPSS 23.0 software package.Values were expressed as mean±standard deviation(?x±s).The correlation between groups was analyzed using t-test.Correlation analysis was performed usingPearson correlation test.P<0.05 indicated statistically significant difference.Results: 1.Compared with the normal control group,the concentration of MIF and TGF-?1 in IgAN patients were significantly increased,and with the increase of pathological grade,the level of TGF-?1 was gradually increased.The difference between the two groups of Lee's classification was different,all had statistical significance(P<0.05).2.There was a positive correlation between blood MIF concentration and TUP in IgAN patients.With the increase of TUP,blood MIF increased(p<0.05),and there was no significant correlation with hematuria,Scr,blood pressure,and glomerular filtration rate.3.The level of TGF-?1 in patients with IgAN was positively correlated with TUP.With the increase of TUP,the blood TGF-?1 was elevated(p<0.05),and there was no significant difference with hematuria,Scr,blood pressure,and glomerular filtration rate.4.There was no significant correlation between the concentration of blood MIF and TGF-?1 in IgAN patients.Conclusions: 1.The expression of serum MIF and TGF-?1 has certain value in predicting IgA nephropathy;2.Serum TGF-?1 expression level can reflect the severity of renal pathological changes to a certain extent;3.There was no obvious synergy between blood MIF and TGF-?1;4.Anti-MIF and anti-TGF-?1 therapy may provide new ideas for the treatment of IgAN.
