Objective: To observe the effect of spleen-kidney-bone tonifying acupuncture on the quadriceps microstructure,minimum fracture force and mechanical properties in the castration-induced osteoporosis model rats in order to explore the mechanism of prevention and treatment of osteoporotic fractures from the perspective of muscle soft tissue by acupuncture.Methods: One hundred and twenty six-month-old SD female rats were randomly divided into model group(n=90)and blank group(n=30).The model group was modeled with castration(resection of bilateral ovaries)and the blank group was resected with a small amount of mesentery near the junction of the ovaries.After 3 months of modeling,model testing was performed.When finished successfully,model rats were randomly divided into treatment group,drug group and blank control group.The treatment group was treated with Spleen-Kidney-Bone Tonifying Acupuncture therapy,the main points are Dazhu,Shenshu and Zusanli,3 times a week for 20 minutes.In the drug group,neostigmine was used.According to the dose conversion principle of 200 ?g/kg,we used neostigmine 0.75 mg combined with 5 ml PBS solution,each subcutaneous injection at 0.1 ml/kg,and treated at the same time with the acupuncture group weekly.The two groups were treated for 6 months totally.The blank group did not use any treatment and was only caught and got back at the same time.After the treatment,A total of 12 rats were taken from half of the two groups,and the left quadriceps femoris of the thighs were fixed at low temperature.Transmission electron microscopy was used to observe the microstructure of the muscle fibers.The right quadriceps femoris and its connection to the upper and lower ends of the femur were clamped and mechanically tested using a mechanical electronic tester.The other half of the 12 rats,their left and right thighs tested in vivo(carried out after abdominal anesthesia),isolated(immediately after completion of the body to break the fresh femur,the femoral neck fractured immediately)the smallest femur fracture by the self-made femur fracture minimal external force detection device.Comparing the test data for statistical analysis,and then based on the results from the role(effect)-function-morphology(microstructure)point of view,we made a comprehensive discussion.Results:(1)Efficacy of external resistance of femurs in each group: In the minimal external force test of femoral fractures,the acupuncture group overall was better than the drug group and the normal group,and the acupuncture group and the drug group were significantly higher than the blank control group.The square test and efficacy evaluation had a chi-square value of 48.851.The difference was significant.There was a significant difference between the acupuncture group and the drug group,comparing with the control group and the normal group statistically(P<0.01,P<0.05).In the minimal external force of isolated femoral fractures,the data in each group was very similar.The Chi-square test showed that the chi-square value of the curative effect evaluation was 0.776.There was no significant difference among the groups,and there was no statistical significance(P>0.05).(2)Comparison of mechanical properties of muscles in each group: In the test of maximum mechanical force and maximum stress of quadriceps femoris muscles,comparisons were made between acupuncture group and drug group compareing with the control group and normal group.The chi-square values of effect evaluation were 29.143 and 23.131,respectively,which showed significant differences(P<0.01,P<0.05).After snapping the rats weight value and calculate the maximum stress in the cross-section of quadriceps numerical data analysis,chi-square value is 0.395 and 0.941,respectively,showed that each group no difference in the weight of rats quadriceps interface under the condition of the solution is also no difference,further reflects the rationality of the accuracy of the data and methods.(3)Comparison of muscle fiber microstructures of each group: In the observation of muscle fiber microstructure,the difference in the connection of myofibrils,Z-rays,and M-wires was observed in the electron microscope images.And we found the difference of the treatment group and the normal group was more significant compared with the drug group and the control group.Obviously,the 3 items of the normal group were well-arranged,neatly-connected.And the acupuncture group was relatively good.The rest have different degrees of alignment and connection dislocation(water ripples).The width of myofibril filaments in the acupuncture group and the drug group was more suitable than the average,and the width and area of the individual sections of each filament were uniform.However,the width and area of the cross section of the normal group and the blank control group were not uniform,and the filament size was different greatly;The myofibrils of acupuncture group and drug group are wider than the normal group and blank control group.In other word,the myofibrils of the normal group and blank control group are small.Conclusion: Acupuncture can increase the maximum load force and maximum stress performance of quadriceps muscle,so that the muscle tissue can increase its direct or indirect buffer absorption effect under the impact of force,and the site is transmitted to the bone tissue under stress.During the process of stress deformation of the bone to break,it plays a very important role in the prevention and treatment of osteoporotic fracture that strengthening the muscle tissue to exert its relative main stress effect or relative cooperative stress effect,so as to increase the fracture threshold of the body's femur against external force.The mechanism of action may be related to the selection of acupuncture points and the exertion of acupuncture techniques,which affected the size of the myofibrillar filaments,Z-wires,and M-wires through indirect or direct pathways such as endocrine and neuromodulation.So that we can make adjustment of the microstructure of the quadriceps muscle fibers.
