BackgroundEpilepsy is a chronic brain dysfunction,The reveal according to statistics from China Association Against Epilepsy,there are about 400,000 new epileptic patients a year,but the prevalence of epilepsy in children was 3.45%.Although most epileptic children can be fully controlled or even cured after formal anti-epileptic treatment,20 to 30 percent of patients still have recurrent seizures after regular drug treatment.Epilepsy recurrent can come to cerebral anoxia and injury,cause sports and intelligence retardation or backward in children,and cause the physical,cognitive,mental,and social function of serious influence,bring a heavy burden to family and society.Therefore,the study of the pathogenesis of epilepsy,the discovery of the evidence of epileptic discharge and the development of targeted drugs are the targets for the control of epileptic seizures.As the study of nerve-immune-endocrine network,which thinking that the occurrence of epilepsy development is closely related to neural immune regulating function disorder,inflammatory cytokine is important carrier of neural immune endocrine network,but the detection of inflammatory cytokines in epilepsy did not cause enough attention to in practical clinical applications.MMP-9 belong to the family of proteolytic enzymes,it can activate other cytokines that there are many studies on the relationship between brain tumor,cerebral edema and autoimmune diseases.The increase of MMP-9 level can promote the formation of inflammatory factors and increase the damage of blood-brain barrier,However,there are few clinical studies on the relationship between positive expression of serum MMP-9 and the pathogenesis of epilepsy immunology.TheTNF-? in the central nervous system is mainly produced by glial cells,glial cells in the brain in children with chronic epilepsy usually appear hyperplasia and activated repeatedly,and cerebrospinal fluid of TNF-? is higher level significantly.IL-6 is a cytokines produced by mononuclear macrophages with a wide range of biological effects,as well as can participate in the immune regulation process of the nervous system.Experimental studies have shown that IL-6 may be used in epileptic cells with classical signal trans duction mechanism,that Participate in the pathogenesis of epilepsy.Therefore,TNF-? and IL-6 may participate in the development of epilepsy.EEG is the spontaneous and rhythmic electrical activity of the brain cells recorded by the electrode,which is an important inspection tool in the diagnosis and treatment of epilepsy.EEG abnormal can reflect the state of nerve lack of oxygen and brain damage,but have a certain sensitivity and specificity,the specific mechanism is not clear,which whether related to the patient's immune status require further research.ObjectiveThe study is exploring the correlation of its in children with epilepsy with Based on the detection of children about newly diagnosed epilepsy of concentration of inflammatory factors,and combined with the degree of abnormal EGG for the pathogenesis,diagnosis,treatment and prognosis of epilepsy.MethodsExperimental group are idiopathic epilepsy 60 cases of new diagnosis based on Collecting in June 2016 to March 2017 in the First Affiliated Hospital of Zhengzhou University of pediatric neurology clinic or hospital children,control group are 30 cases of normal children at the same time the hospital physical examination.60 cases of children with epilepsy were extracted 2 ml peripheral blood in recent attack,leting it stand and centrifuge,keeping the serum in the EP tube,save to-80 ?refrigerator.Groups epilepsy children and normal children was detected the serum of MMP-9,TNF-a,IL-6 level using ELISA method.EEG was used for dynamicEEG examination of our hospital.Using SPSS24.0 statistical software for the statistical analysis on the experimental data.Results1.The levels of MMP-9,IL-6 and TNF-? were compared in epilepsy subgroup and control group.The levels of MMP-9,IL-6 and TNF-? in the epilepsy subgroups were significantly higher than those in the normal control group,the difference was statistically significant(P <0.05);The levels of MMP-9,IL-6 and TNF-? in the seizure status group and frequent group were significantly higher than those in non-frequent non-persistent state group,the difference was statistically significant(P<0.05);The levels of TNF-? in the seizure status group were significantly higher than those in the frequent group,the difference was statistically significant(P<0.05),The levels of MMP-9 and IL-6 in the seizure status group were not significantly higher than those in the frequent group,the difference was not statistically significant(P >0.05).2.EEG results were compared with MMP-9,TNF-? and IL-6 levels.The levels of inflammatory cytokines were gradually increased in normal EEG group,diffuse slow-wave EEG group and epileptic wave EEG group;The levels of MMP-9,IL-6 and TNF-? in diffuse slow-wave EEG group and epileptic wave EEG group were significantly higher than those in normal EEG group,the difference was statistically significant(P <0.05);The levels of MMP-9,IL-6 and TNF-? in epileptic wave EEG group were significantly higher than those in diffuse slow-wave EEG group,the difference was statistically significant(P <0.05).3.Correlation analysis of TNF-a or IL-6 and MMP-9 in serumThere was a significant correlation between TNF-? and MMP-9 levels in serum,and the concentration of TNF-? was the ordinate,the concentration of MMP-9 was horizontal,It can be seen that the two are positively correlated linearly from the scatter diagram,pearson correlation analysis was further performed,correlation coefficient r2=0.752,p<0.05.There was a significant correlation between IL-6 andMMP-9 levels in serum,and the concentration of IL-6was the ordinate,the concentration of MMP-9 was horizontal,It can be seen that the two are positively correlated linearly from the scatter diagram,pearson rank correlation analysis was further performed,correlation coefficient r2=0.680,p<0.05Conclution1.Immune inflammation may be involved in the occurrence of epilepsy,This view can provide new targets for epilepsy treatment.2.EEG abnormality can be related to cytokine level,and cytokines can be used as an auxiliary diagnostic indicator for EEG.
