Objective To study the effect of using artificial dermis and autologous split thickness skin graft with vacuum technique in the treatment of deeply electrical injured wounds.Methods From January 2011 to January 2013,30 cases of electrical injured patients,randomly divided into two groups.15 patients with 20 wounds combined negative pressure technology in the treatment group,15 patients with 20 wounds in the control group.The wound area of the two groups was 0.5-4%(?°-?°),and the wound site was equivalent.After debridement,two operations were carried out.During the first one,artificial dermis was impl anted.During the second one,artificial dermis was transplanted with auto logous split thickness skin graft.Negative pressure suction technique was given to the treatment group in both operations while the control group not.Both groups were treated with anti-inflammation,dressing change,anti-scarring and functional exercise,and followed up for 1 year.The survival rate of skin grafts and the quality of wound healing in the two groups were compared.Results The treatment group was able to have the se cond operation while angiogenesis occured 5-7 days after the first one and the wound was closed 7-10 days after skingraft.Following up for 1 year,skin elasticity,abrasion resistance and joint activity turned to be good,scar is not obvious.Patients in control group received second operation 10-14 days after the first one and wound was closed in 10-16 days after surgery.The healed sites comparatively showed darker color,less motor activity,prone to ulceration,remarkable scar.There is no significant d ifference on infection between two groups.3 cases of infection occurred in treatment group after the first opration while no more infection happened after the second one.In control group,6 cases of infection occured after the first operation,and of four after the second one.Conclusion Artificial dermis,autologous split thickness skin graft combined with vacuum technique in the treatment of electrical injury deep wounds can increase the survival rate of skin grafts,shorten the duration of hospital sta y,and improve theprognosis.
